📝 “What are Important Adverse Effects of Common IV Sedations Medications used in Dentistry?”
from Dr. Gallagher's Podcast · host Brendan Gallagher, DDS
- 9.18.24 Quick Review #229 - #dental #dentist #dentistry #dentalstudent #dentalschool #teeth #cavity #wisdomteeth #extraction #oralsurgery #oralsurgeon #anesthesia #IVS #sedation #IV #macsedationAmong the commonly ued IV sedation medications — propofol, fentanyl, ketamine, midazolam (versed), and dexmedetomidine (precedex) — adverse effects vary in frequency and severity.1 - Propofol has a well-documented association with significant adverse effects, especially respiratory depression (up to 60.7% in some studies), hypotension, and in rare cases, propofol infusion syndrome (PRIS), a potentially fatal condition . These effects are dose-dependent and can escalate with prolonged use or in critically ill patients.2 - Fentanyl, a potent opioid, also presents serious risks, including respiratory depression (which can occur in up to 30% of cases), bradycardia, and hypotension . However, compared to propofol, it is less likely to cause hypotension but has a high risk of hypoxemia when used for deep sedation .3 - Ketamine, while generally safer regarding respiratory function, can induce emergence reactions (e.g., hallucinations) in 10-20% of cases and cause mild increases in blood pressure and heart rate .4 - Midazolam (versed) is often used for anxiolysis and sedation but can cause respiratory depression in around 10-20% of patients when combined with other sedatives or opioids like fentanyl.5 - Dexmedetomidine (precedex) is favored for its lower risk of respiratory depression but can still cause bradycardia and hypotension, with reported incidences up to 30-40% depending on the dosage and duration .While all medications have significant adverse effects, propofol tends to show a higher incidence of serious complications like respiratory depression compared to others like dexmedetomidine or ketamine, especially in higher doses or prolonged use.References:1.
