Additionally, alcohol interferes with the absorption of thiamine in the gastrointestinal tract and affects the liver’s ability to store and utilize thiamine. Progression to Serious Conditions: 1. Wernicke’s Encephalopathy: • Acute Phase: Wernicke’s encephalopathy is an acute neurological disorder resulting from severe thiamine deficiency. It is characterized by a classic triad of symptoms: ophthalmoplegia (paralysis or abnormal eye movements), ataxia (lack of coordination), and confusion. However, not all patients present with all three symptoms. • Pathophysiology: Thiamine is a critical cofactor for several enzymes involved in glucose metabolism. Its deficiency leads to impaired cerebral energy metabolism, causing neuronal damage, particularly in regions like the mammillary bodies, thalamus, and brainstem. • Treatment: Immediate administration of thiamine is crucial. If treated promptly, Wernicke’s encephalopathy symptoms can be reversed. However, if left untreated, it can progress to Korsakoff syndrome. 2. Korsakoff’s Syndrome: • Chronic Phase: Korsakoff’s syndrome is a chronic, debilitating condition that often follows untreated or inadequately treated Wernicke’s encephalopathy. It is characterized by severe memory impairment, confabulation (making up stories to fill memory gaps), and learning difficulties. • Pathophysiology: Persistent thiamine deficiency leads to irreversible damage to the brain’s mammillary bodies, thalamus, and other areas critical for memory and cognitive function. This damage results in the hallmark symptoms of Korsakoff’s syndrome. • Management: The condition is largely irreversible. Management focuses on supportive care and ensuring adequate thiamine intake to prevent further deterioration.
