What's your favorite thing to do at Lynchburg?Shawne Farmer: Go to the restaurants probably.Cami: Yes, we've got some good ones.Shawne Farmer: We do have fabulous restaurants. For sure.Cami: It's become this fun place to go walk around downtown and experience some of the local flair. Lynchburg is coming into its own and I love seeing that happen kind of in each of our communities that we have around us. So you've been with Interfaith Outreach for 12 years. What brought you to Interfaith?Shawne Farmer: Well, I had spent some time in nonprofits before at Thomas Jefferson's Poplar Forest and Interfaith Outreach. And then I had also taught in the school system as well. And I just, after working with a lot of children and in nonprofit, I thought it would be great to work for a nonprofit that could help families, support families that could empower children to be all they can be.Cami: Oh, I love that. So for those who have never heard of Interfaith, tell us what is at the heart and the mission of what you guys do?Shawne Farmer: We have six programs that pull the resources together from the community to keep families together safe at home.Cami: So what do those programs look like?Shawne Farmer: We have an emergency assistance program where we offer assistance with rent and utilities. We have a progressive release program where we go into the jails and teach life skills classes to men and women. We also offer aftercare when they get out so they can be successful. We have a free furniture program for families that are displaced or who have been homeless. We have a vision program where we provide rides to medical appointments for people who are blind. We have a rebuilds program where we do simple home repairs for elderly and disabled people. My name is Cami Smith and I am your host. And I am here today with Dr. Travis Engel, who is the chair of pediatrics here at Centra. And we are going to dive into the fact, the fiction, all of the things we need to know about RSV. Because it is sweeping across not just our community, it's sweeping across the nation. We're kind of at a critical point. And so we're going to have you share all of your wisdom with us. But before we dig into that, why don't you tell us a little bit about yourself and what brought you here to Centra?Dr. Travis Engel:Yeah, absolutely. So my wife and I, and our three kids actually relocated back in July for this position. I graduated in Liberty University's first medical school class.Cami Smith:Very cool.Dr. Travis Engel:So I was here from 2014 to 2018, and we always knew this was a place we kind of wanted to come back to. After that, briefly stopped in Chicago for three years and did my residency there in Northern Chicago, which was a great experience. And then I have some additional fellowship training in pediatric critical care, which I got at Children's Mercy in Kansas City Missouri, which is an amazing hospital. So the ability to wrap that experience back up and come back here and work here in the community that really kind of started my medical education is really neat and it's an exciting time for our family.Cami Smith:You love the Lynchburg area. Great place for our family. What are some of your favorite things about Lynchburg?Dr. Travis Engel:Oh my goodness. The view. I mean, I always tell people there's really just not a bad place to look in the city.Cami Smith:There's not.Dr. Travis Engel:And also getting to really reengage with the medical students over at Liberty and kind of start giving back to that school that, like I said, launched my medical career has been really neat.Cami Smith:So do you go back? Oh, that's right. You said you teach there.Dr. Travis Engel:Yeah, so I'm actually adjunct faculty over at the medical school now, so it's really kind of come full circle for us. It's been really great. And I think a lot of people don't realize here in the community that we actually have kind of an academic medical center. I mean, yeah, we really do. We have medical students, students, nursing students, and we're doing a lot of medical education there.Cami Smith:That's really cool. So why Centra?Dr. Travis Engel:Well, I mean, number one, the area. I think we wanted to come back to this area. Number two, just I would really emphasize the culture of yes there. There's a lot of programs and a lot of things I wanted to do when I came back to Centra and a lot of things I wanted to bring back with me. And the organization as a whole has been super enthusiastic about engaging on all fronts in addition to the fact that I think there is such a community need to have a pediatric resource like Centra for pediatric inpatient care. Here so that families don't have to go 45 minutes to an hour away to get pediatric care. So that's really important to me, keeping families close to home, getting them better here, not sending them an hour away for services that we can offer easily in this community.Cami Smith:Yeah, I love hearing your passion behind that. And so today I have with me Debbie and Brenda. And Brenda is actually a nurse practitioner, correct?Brenda:Correct.Cami Smith:Okay. A neonatal nurse practitioner.Brenda:Yes.Cami Smith:And then Debbie is a bedside nurse, registered nurse over in the intensive care unit. Correct?Debbie:Yes.Cami Smith:Okay. So I'm so excited that you guys have taken your time to come and share your story. I mean, I'm assuming you haven't been there for 50 years. Neither of you look old enough to be there for 50 years, but the time that you have spent there I'm sure has made such a huge impact on you. It's such a special place. So we'll start with Brenda. Can you just tell us a little bit about yourself, your position, what you do here at Centra?Brenda:Okay. My history with Centra and Virginia Baptist Hospital has been longstanding. Way back, I was actually born at Virginia Baptist Hospital and Dr. Peter Houck, who pioneered our unit and started it in October 2nd of 1972, was actually my private pediatrician as a child.Cami Smith:I love that so much.Brenda:So little did I know then that his work and efforts would influence my own career path. So I graduated from Lynchburg College, now called the University of Lynchburg and was employed by Centra in May of 1984. At that time there were no positions available in our intensive care nursery. So I was hired for coronary care unit at Virginia Baptist Hospital. I did speak directly with the CEO of Centra at that time, because I was very interested in having a position in the intensive care nursery and he promised me that if a position became available that they would transfer me.Cami Smith:Wow.Brenda:So he actually kept his promise and I was only two months into my orientation in July of 1984 that I was transferred to the intensive care nursery and I have been there ever since.Cami Smith:Okay. What was that like to step into this place you've wanted to work for so long, somewhere in a place where you were born? What was that like to start on that first day?Brenda:Scary. I knew I had only had one day of orientation as a nursing student in the intensive care nursery and I knew that it was a different field of medicine from any other field that existed and nothing that I got any training on or education with in school. But I just knew that I was better suited for the tiniest of patients. Adults were just too big for me. Coronary care unit was scarier than the intensive care nursery.Cami Smith:I can imagine. Okay. Debbie, why don't you tell us a little bit about yourself and how you came to work at Centra?Debbie:Well, I graduated in 1985 from the Lynchburg General Hospital School of Nursing, which is now Centra School of Nursing. And I thought I wanted to work with babies in the newborn nursery. I had no orientation to the intensive care nursery at all, so I didn't know much about that. But unlike Brenda, they had no positions available anywhere except for in the intensive care nursery. And so, Donna has been driving this Mammo Van for...Donna Lloyd:21 years.Cami Smith:21 years. And so, she plays such a large role as women come in. So me, I had my first mammogram for the first time ever today, and you were the first person I saw. You helped me feel comfortable, you helped me feel at ease. You play a big role in this.Donna Lloyd:Yes, I do.Cami Smith:I want you to tell us a little bit about that. But first, tell us a little bit about you. How did you come to be a part of the Mammo Van unit?Donna Lloyd:Actually, I drove a school bus for Campbell County four years prior to that.Cami Smith:Oh, okay.Donna Lloyd:But I've been with Centra off and on since I was in high school. I started out in radiology as a receptionist.Cami Smith:Oh, okay.Donna Lloyd:Yeah. And then I finished high school, and then I started driving a school bus and I saw this.Cami Smith:Yeah.Donna Lloyd:So I applied for it and I got it.Cami Smith:I love it.Donna Lloyd:We used to have an old, it was a box, more like a box truck.Cami Smith:Like a trailer?Donna Lloyd:No, it was like-Cami Smith:Oh, like the Pace vans.Donna Lloyd:Yes. It's a box truck.Cami Smith:Yes.Donna Lloyd:I would drive, sit up front, and then I'd have to get outside to go to the back. But here, I could just walk.Cami Smith:Yeah. This is very nice.Donna Lloyd:We've had this since 2016.Cami Smith:Oh, wow.Donna Lloyd:Yeah.Cami Smith:So what would a woman do, how do you get an appointment for the Mamm Van?Donna Lloyd:It just depends. We go to a lot of the outlying communities. They want the dates, we give them the dates, and we send them schedules.Cami Smith:Okay.Donna Lloyd:And once a month, we go to a lot of places once a month.Cami Smith:Okay.Donna Lloyd:And businesses, we go to businesses and same thing, they've got a certain person that will take care of that schedule.Cami Smith:Got you.Donna Lloyd:They'll get all the information from the patients. And a week prior to them coming to us, we call to get an updated schedule. So, once the day we come, we go to that site, I'll go in and I'll do everything that I've got to do and then they just have their appointment times and they just come on that day.Cami Smith:Okay.Donna Lloyd:Does that make sense?Cami Smith:Yeah, absolutely. And it's so convenient. So, this van right now is in the parking lot of the building where I work. I am your host, Cami Smith, and I am here with Dr. Pavalonis, who is a vascular surgeon here at Centra. The reason we're here discussing today is because the month of September is peripheral vascular disease awareness month, and so we want to bring some highlight, some awareness around what that is because I know, for me, this is not something I have encountered in my personal life, and maybe those of you who are listening are not aware of what that is or how it's impacting maybe even those around you. And so we're going to go ahead and jump in. Dr. Pavalonis, thank you so much for being here today and for having this conversation with us. Why don't you tell us a little bit about yourself first, before we jump in?Dr. Pavalonis:Well, I'm a vascular surgeon here in Lynchburg, Virginia. I love being here. This is my third year of practice within our fine city. We have a very busy practice. Peripheral vascular disease is very prevalent in this area and something that we have a lot of work to do on.Cami Smith:That's incredible. See, I had no idea that it was so prevalent. Explain to us what exactly is peripheral vascular disease?Dr. Pavalonis:Peripheral vascular disease is impaired blood flow primarily to the lower extremities, mainly in the legs, that can cause all sorts of uncomfortable and potentially life threatening problems such as limb loss, lifestyle limiting factors, and a whole host of other things. It can also be indicative of other more serious problems like heart disease, cerebral vascular disease, stroke type things.Cami Smith:Goodness, so not only could it be something that somebody just has that they're walking through as a disease in their life, but it's also an indication of maybe something a little bit deeper.Dr. Pavalonis:Correct.Cami Smith:Okay. Is this something that they're born with, that they have, it's in their genetics, or is this something that, like you said, can come at later, or both?Dr. Pavalonis:It really seems to be a lot of lifestyle-type decisions and problems, but there's also a hereditary component as well that can affect some people. Taking control of diabetes, high blood pressure, quitting smoking, modifiable-type factors can be very helpful in potentially avoiding this, but then there are also some hereditary issues that folks can suffer with and can develop this through sometimes no fault of their own.Cami Smith:So interesting. I feel like we have conversations about a lot of the things that we treat here at Centra, and so many of them come back to lifestyle, come back to things like smoking, come back to things like eating healthy, getting up and exercising. It really blows my mind how much just having an active and healthy lifestyle can clear up some of these things that can creep in if you're not careful.Dr. Pavalonis:Oh, absolutely.Cami Smith:Would you agree?Dr. Pavalonis:Oh yes. A lot of these things, just watching what you eat, regularly following up with your doctor, not smoking, those types of things are very, very important in avoiding this type of problem with peripheral vascular disease.Cami Smith:Okay. So what are some symptoms? I am here with a team from our Centra Bridges Treatment Center and we're celebrating the 35th anniversary. When is the 35th anniversary?Dr. John Hendrickson:I guess, technically, it was in June.Cami Smith:It was in June, but we're celebrating all year long.Dr. John Hendrickson:Right. We're going to, I think, have a celebration sometime in August.Cami Smith:I love that. Okay. So as we're anticipating the celebration, we want to give you a little bit of an inside view of the history of Bridges, some of the big moments in accomplishments and amazing things that have been accomplished treating children who have gone through traumatic experiences. But you all can speak to that much better than I can, so I'm going to let them introduce themselves and we'll start on the end with Dr. John Hendrickson.Dr. John Hendrickson:Hi, I'm John Hendrickson. I'm the medical director. I'm a child and adolescent psychiatrist at Bridges Treatment Center. I joined Centra in 1997, so I recently celebrated my 25th anniversary with Bridges. And when I joined in '97, Bridges was 10 years old and, at that time, was just opening its third cottage, which was known as Ivy Cottage. And so, I joined Dr. Shilling, the previous medical director, to try to expand the census there at that time. Subsequently, we got as big as five cottages and 102 beds. We're working considerably smaller than that presently. But that's my story and how long I've been there. I have not been there as long as these other two gentlemen, so they can tell you more of the early history than I can.Cami Smith:All right.Cray Callahan:Wow, early history. I'm Cray Callahan, the principal of the school program at Bridges. Obviously, the kids live there and they have to go to school there. I started there in January 1990, actually working in one of the two cottages at that time. We only had Chimney and Cedar, and I was working third shift, working on my master's in special education. And I did not know that I would stay at Bridges that long after I completed my master's, but a teaching position became available. The director at the time said, "I hear you have a teaching certification." And I said, Yes," and I rolled into a position and have been there ever since.Cami Smith:The rest is history. Wow. All right, and Mark.Mark Bosher:Yeah, I'm Mark Bosher. I started at Bridges back in December 1992, but I came for a little bit different reason. I loved fishing and I called Aaron Ruley up, who was one of the therapists at Bridges at the time. And he was getting ready to leave for Michigan, and I said, "Do you guys have a place there?" So I jumped on board and started fishing at the lake, and really enjoyed that whole feature of being able to get some of my dreams met. I started as a therapist doing individual therapy, as well as family therapy. And it has been a love of my life. I've really enjoyed working at Bridges the whole entire time. I've got lots of different aspects that we did while we were there.              When I started, we were just leaving from a substance abuse program, so I started doing group with kids that had codependency and dual diagnosed, and that was very challenging to do that. I'd worked up in Charlottesville, in a hospital that treated the same types of kids as well. So today, I am here with Krista Leake and she is a physical therapist. We're going to be talking about concussions, something that I would say many people have had experience with, especially in this season. We're headed into fall. We're headed into the school year, the new sports season, where there is a lot of knocking around happening.Cami Smith:So for parents, but also for athletes and for just the public in general, it's so important to know the things that will help you that can be preventative, but then also for care after the fact. So Krista, I want you to tell us a little bit about your role here at Centra. Before we jump into all of that, how did you get into this field?Krista Leake:Well, by trade I'm a physical therapist and I'm also a certified athletic trainer. So both of those are involved in concussion care. But my role at Centra, I am the clinical coordinator of quality. So I really focus on making sure that everyone is performing evidence-based treatment and that we are giving the best quality care that we can.Cami Smith:I love that. So what is a concussion, like a clinical perspective? Because I know when I hear a concussion, I hear somebody got hit in the head and you need to be very careful. But from a clinical perspective, how can you define that for us?Krista Leake:So the official definition of a concussion is that it's a type of a traumatic brain injury. It is caused by a bump, a blow or a jolt to the head, or by a hit to the body that can actually cause the head to shake and the brain will either go forward or back side to side or even turn inside of the skull.Cami Smith:Oh, okay.Krista Leake:That sudden movement can cause the brain to bounce around, twist and then it causes a chemical reaction.Cami Smith:Oh wow.Krista Leake:Sometimes it can even some stretching or damaging to the brain cells. But the important thing is is that chemical reaction. It's not where you get a severe traumatic brain injury where you may have a bleed in the brain, which is a structural thing. This is something that's very difficult to see. Sometimes I like to explain it like it's a sprained brain versus a bruised brain or a cracked skull. What's interesting, it is kind of an invisible illness. You can't look at someone, say, "Oh, you have a concussion." So it is very much a clinical diagnosis. We go by signs, we go by symptoms and how someone is feeling. There are imaging that you can do. They rarely show anything. [inaudible 00:02:57].Cami Smith:Yeah. That was so surprising to me when I saw that an MRI or a CT scan, this will not detect a concussion.Krista Leake:Right. Occasionally, if it's super severe, you may see some changes on there.
