In light of recent public discussion surrounding Operation Hummingbird, including criticism raised by politicians and commentators regarding the handling of the investigation, I would be grateful if the following questions could be put to DCS Hughes Cheshire Police.The purpose of these questions is to seek clarity regarding investigative procedures, evidential standards, expert selection, media conduct, and wider issues of public accountability.Initial Decision to Launch the InvestigationFollowing relatively brief discussions with Dr Stephen Brearey and Dr Ravi Jayaram, what evidence was considered sufficient to justify the launch of a major criminal investigation into alleged murders?The initial decision to launch a major criminal investigation (Operation Hummingbird) in May 2017 was triggered by a meeting between Cheshire Police and consultants Dr Stephen Brearey and Dr Ravi Jayaram. While hospital management had previously resisted, the police deemed the evidence presented—primarily focused on an "unexplained and unexpected" surge in baby deaths and collapses—sufficient to warrant a formal investigation.The evidence presented by the consultants in 2017 included the following key elements:The "Common Link": The consultants presented data indicating that nearly all the suspicious neonatal deaths and collapses occurred on shifts worked by one nurse—Lucy Letby.A "Pattern of Emergencies": Following a shift change, the consultants observed that the cluster of unexplained emergencies moved from night shifts to day shifts, matching Letby's new rota.Specific Clinical Incidents: Dr Jayaram informed the police of an incident in February 2016 where he found Letby standing over a deteriorating, premature baby with a dislodged breathing tube, failing to take action.Failed Internal Investigations: The consultants informed police that previous reviews—including a thematic review of neonatal mortality conducted in February 2016—had failed to explain the rising mortality rates.Evidence of Poisoning: Dr Brearey later cited the discovery of a blood test from 2015 showing extremely high insulin levels, which he believed provided evidence of intentional poisoning.Shift in Management Position:Although doctors had been raising these concerns since at least June 2015, hospital management initially dismissed them, even forcing consultants to apologize to Letby in early 2017. He also later stated in court that he would not describe himself as an “expert,” despite acting professionally as an expert witness following retirement.Given the seriousness and complexity of the allegations, why was reliance placed so heavily upon a single expert rather than a broader panel of recognised neonatal specialists?Was this considered best investigative practice in a case involving highly specialised neonatal medicine?In addition, before appointing Dr Evans, did Cheshire Constabulary carry out any background review into his publicly expressed views, published commentary, or previous conduct as an expert witness?There are publicly available reports and online material alleging that Dr Evans previously expressed controversial views regarding the decriminalisation of certain sexual acts involving minors. In this context, it is very significant that Robert, Diane and Martin Hofschröer have never given any justification as to what claim he has against his mother’s savings, or why she should hand them over to him, or what he wants to do with them when he gets them. Letter Demanding money with menaces11 11.07.2008 Diane sends abusive e-mail to Peter, stating she wished Peter would return to Austria. e-mail Harassment of Barbara’s carer to make him leave and abandon control of the savings to Robert, Diane and Martin. 112 ??.08.2008 It is clear to Peter that his parents home 74 Rosedale Avenue, which has only one toilet upstairs is completely unsuitable for the needs of his parents, who are both incontinent and they need to move. Peter finds suitable sheltered accommodation for his parents at Fairfax Court and after discussion wit his parents, puts 74 Rosedale Avenue on the market to finance the purchase of the flat. The flat is half way between Robert’s house and Diane’s house and Peter is still hoping they will play a role in the care of Paul and Barbara. Robert contacts Halifax estate agents without reference to his parents to block the sale of house, thereby preventing the financing of suitable sheltered accommodation for Peter and Barbara. Robert avoids eye-contact and covers his face with his hand. He states that all the conversation should be directed through Diane. Robert demands to know where Mum has her savings. Diane is abusive, screaming that she hates Peter. She is so agitated she broke the chair she was sitting on. They nevertheless agree to let the sale of the house go ahead. He says the social worker's reply came without a moment's hesitation: "You'll be prosecuted for child abandonment."Verity and Ian are just two of 50 parents the BBC has spoken to during a six-month investigation into why adoptive families are being broken apart and their children returned to care.It has uncovered a hidden world where parents are subjected to lies and blame by authorities across the UK, as they struggle with the needs of traumatised children who have often suffered abuse and neglect before they were removed from their birth families.Parents speak of being threatened, harassed and pushed into mental health crisis, while one adopted teenager who returned to care says that with more support his family might still be together now.Warning - this story contains discussion of suicideThe BBC conducted the most extensive Freedom of Information request ever into adoptions that have broken down, finding that more than 1,000 adopted children in the UK have returned to care in the past five years. But they describe his teenage years as a "living hell" and they feared for their safety as he would threaten them with knives.One morning, Verity left a note for Ian and drove to the coast with the intention of killing herself. A passer-by spoke to her, which she says saved her life.On another occasion, after one of Liam's meltdowns, his parents called the police as they had been advised to in a situation like this. But it was Verity who was arrested, after Liam accused her of assault."It's the worst experience I've ever had. "She came up to me and she was like, 'I've just had a call with your social worker and you can't go home'," he told the BBC."And just hearing that, my heart just broke."Verity says: "It was awful. We were forced into doing it this way, and essentially making him homeless, in order for the local authority to get their acts together."Now aged 17, settled and living independently after several previous moves, Liam takes responsibility for his behaviour, but says it is "a shame that it's impacted what I call a family". He says local authorities should have an option to voluntarily accommodate a child that would be considered beyond the parent's control, without the presumed need for evidence of blame.'Desperate for help'Verity and Ian were among dozens of families the BBC met in Manchester, at an event organised by a group called Patch - which stands for Passionate Adopters Targeting Change with Hope - whose members are adoptive families challenging what they see as fundamental failings in the system."Adoption is lifelong, so must be the support," says Jackie from Essex, one of the parents.Fiona Wells, who is adopted herself and a social worker, set up the group after she faced huge issues with her three adopted children.
