Skrmetti, I invite you to consider the recent court ruling through a medical lens. Perfect or Banned? The Double Standard in Gender-Affirming Care (65)

Why is gender-affirming care expected to be flawless when no other area of medicine is? In this episode, we unpack what “perfection” really means in this context—the idea that even one bad outcome is treated not as an individual case to learn from, but as evidence that the entire system is broken. In most areas of medicine, unusual outcomes are examined as outliers. But with gender-affirming care, those rare exceptions often overshadow the thousands of positive, life-saving experiences. We explore how that double standard fuels feEnjoyed this episode? Many of these stories aren’t just misleading—they’re factually incorrect. A Missing Perspective: Adults Who Transitioned in Childhood

Are you prepared to treat transgender adults who transitioned before puberty? Most conversations about transgender healthcare focus on those who transition after puberty—but what about those who transition earlier in life? The first generation of transgender people who transitioned in childhood are now adults, yet many medical providers have never considered how their experiences and healthcare needs might differ. In this episode, we begin a critical conversation about this often-overlooked perspective. What does it mean for a patient to have lived their entire adolescent and adult life in alignment with their gender? How might their medical, social, and psychological experiences differ from those who transition later? And why is it so important for providers to understand these differences? This episode kicks off a multi-part series exploring the unique aspects of transitioning in childhood—covering social, medical, and surgical considerations. The NDAA and Gender-Affirming Care for Minors: A Political Battleground

Congress is doing something unprecedented—legislating healthcare coverage for TRICARE in a way it never has before. In this episode, I discuss H.R. 5009, the National Defense Authorization Act (NDAA), which includes a political amendment in Section 708 that directly affects patient care. This amendment, buried within a crucial bill funding our nation’s military defense, prohibits TRICARE from covering specific gender-affirming care for minors. What does this mean for military families? And what are the broader implications for healthcare and patient rights when political amendments shape medical decisions? These questions weigh heavily on my mind as I explore the language of the bill, the medical concerns it raises, and the precedent it sets for future legislation.I challenge you to step outside your comfort zone and explore a perspective you might not fully agree with. It’s okay to disagree—what matters is staying curious and open enough to listen without immediately shutting it down. Let’s commit to understanding, even if just for a moment, a different way of thinking.Take the challenge. Share the episode. And let’s stay curious—because awareness matters. If you want to read the bill, you can find it here. Enjoyed this episode? Don't Believe Everything You Hear: How to Evaluate Gender Expertise (#45)

In this episode of Gender Matters: Insights from a Physician, we explore a challenge many face today: how to critically evaluate what we hear about gender from people we assume are “experts.” Just because someone holds a specific title or shares a personal connection to the topic doesn’t mean their perspective is comprehensive—or even accurate.Join me as I share a fascinating story from a transgender conference that illustrates why one perspective isn’t universal, and learn how to ask the right questions to uncover where someone’s expertise truly lies. We'll also tackle the common human tendency to embrace voices that affirm our beliefs without fully understanding their background. This episode challenges listeners to think critically and offers actionable steps for navigating expertise—without giving away too much.Listen now to gain a better understanding of how to evaluate "experts" critically.Enjoyed this episode? If 97% is Good Enough for Cardiac Patients, Why Isn’t It for Transgender Youth?

Why do we accept a 97% success rate as a gold standard for cardiac care yet question it for transgender youth who receive gender-affirming care? In this episode, Dr. Panszi explores a pivotal new study from JAMA Pediatrics that examines satisfaction, regret, and continuation of care rates among transgender youth who received their gender-affirming care at the start of puberty. At 97%, the continuation rate mirrors the outcome for cardiac patients who experience improved quality of life after surgery. Yet, while cardiac patients face a 3% mortality rate, those receiving gender-affirming care simply discontinue care. The distinction raises essential questions about how we view and measure success in healthcare, especially when the patient population is young and transgender.Listen in to understand the facts behind these numbers, the factors contributing to quality of life improvements, and how this evidence can empower healthcare providers to better support and counsel their patients.Citations:Olson KR, Raber GF, Gallagher NM. Why Correcting Misinformation on Gender Care Isn’t Political

As physicians, we know there’s an unwritten rule: don’t discuss politics or religion with patients. But when it comes to correcting misinformation, especially about gender-affirming care, this is not a political act—it’s our responsibility. In this episode, I explore why some providers shy away from addressing myths and misconceptions, how this confusion is affecting our practice, and why it’s time to step up.When misinformation spreads unchecked, it shapes public perception, and ultimately, legislation that restricts our ability to deliver the best care. We already debunk myths daily about medications, treatments, and health conditions—so why treat gender care any differently?Listen in to learn why facts matter, what role we can play, and how to shift the narrative, empowering patients and society with accurate, reliable information.Enjoyed this episode?
