This is not a takedown of polyvagal theory. It is an invitation to think carefully about what we actually know, what we claim to clients, and how to hold a clinical heuristic and a neurobiological claim at the same time. Polyvagal theory has become a shared language across trauma, grief, somatic, and relational therapy — and that language gives clients a way to understand their own experience, which matters. But the framework rests on specific neurobiological claims, and those claims are contested. Polyvagal theory identifies RSA as the primary marker of ventral vagal activity and, by extension, of the capacity for social engagement and self-regulation (Porges, 1995, 2001). Higher RSA is generally associated with better cardiovascular health, stress recovery, and social engagement capacity, and is one measure of heart rate variability (HRV). Polyvagal Theory — The Three-State Model Porges's (1995) hierarchical model of autonomic nervous system function, proposing three evolutionarily layered states: ventral vagal (social engagement, safety), sympathetic activation (mobilization, fight/flight), and dorsal vagal (immobilization, shutdown, freeze). The model is foundational to trauma and somatic therapies and has shaped clinical language around co-regulation and neuroception (Porges, 2011). The Grossman Critique Grossman and Taylor (2007) and subsequent commentary raise concerns about the neuroanatomical precision of polyvagal claims — specifically whether RSA reliably indexes a distinct myelinated vagal pathway, and whether the dorsal vagal shutdown state is anatomically and functionally distinct in the way the three-state model proposes. The critique does not reject the clinical value of polyvagal-informed practice; it calls for greater precision in how neurobiological claims are framed and communicated. Clinical Heuristic vs. Neurobiological Claim A clinical heuristic is a framework that helps clinicians and clients organize experience — it is useful when it guides observation, language, and intervention, even if the underlying mechanism is not fully established. A neurobiological claim is a specific assertion about how the brain and body actually work. SHOW NOTES | EPISODE EP 7 The Polyvagal Theory Debate, Part 1 of 3 What Actually Happened — The Critique, the Response, and What "Untenable" Actually Means Host: Kathy Couch, LCSW | March 2026 EPISODE SUMMARY If you've been in any trauma-informed clinical space in the last ten years, you've almost certainly built some part of your practice on polyvagal theory. And then maybe recently you heard that 39 scientists published a paper calling it scientifically untenable — and you thought, wait, what? That was my reaction too. And I think for a lot of clinicians, it landed somewhere between confusing and destabilizing. Do I need to throw out the whole framework? The answer is no. But the conversation is worth having — carefully, honestly, and without either defending the theory or reflexively dismissing it because the headline was alarming. That's what this three-episode series is for. In Part 1, we break down what actually happened, what each side argued, and what the word "untenable" actually means when scientists use it. Grossman and colleagues are not arguing that co-regulation is a myth, that neuroception doesn't matter, or that nervous system states are irrelevant to clinical work. The debate is about specific neuroanatomical claims — specifically, whether respiratory sinus arrhythmia (RSA) can function as a selective index of ventral vagal activity the way the theory requires. RSA is the natural rhythm of your heart rate tracking with your breath. When you inhale, your heart rate speeds up slightly. When you exhale, it slows down. That fluctuation is RSA — it's measurable, it's been studied for decades, and polyvagal theory assigns it a specific role as a marker of ventral vagal regulation. The critics say the anatomy doesn't support that level of specificity. What Porges argued in response: that the critics mischaracterized his claims and engaged with a more rigid version of the theory than he proposed — and that RSA as a clinical heuristic is defensible even where the precise anatomy remains contested. The critics' response: the version clinicians are actually using is the one we critiqued. Both things can be true. And that complexity is exactly why this conversation belongs in a clinical education space — because how the framework has been taught is part of what the debate is about. WHAT IS NOT IN DISPUTE Safety matters. Co-regulation is real. Nervous system state shapes what's possible in the clinical encounter. None of that is in question. Clinicians can continue to build on those foundations while the mechanistic story underneath them gets refined. In This Week's Episode Meet Messina Delgado, LPCA Messina Delgado is a provisionally licensed counselor based in Sugarland, Texas, where she practices at Open Narrative Counseling and Robin Lee Counseling. She specializes in working with trauma and neurodivergence across the lifespan, serving both children and adults with a deep understanding that "adults are tidily larger children, especially when it comes to sensitive materials." As a neurodivergent clinician herself, Messina brings lived experience and authentic insight to her therapeutic work. Her journey to becoming a therapist was shaped by years of being undiagnosed, navigating expansive interests, and ultimately learning to embrace observation as a powerful tool for understanding both self and others. Messina is passionate about adapting EMDR therapy for neurodivergent populations, emphasizing the importance of trusting clients' experiences, creating sensory-safe environments, and honoring each person's unique way of processing. She incorporates creative approaches including art, visual tools, somatic work, and parts work to meet clients where they are. Her philosophy centers on the belief that "behavior is conversation" and that attunement is imperative—recognizing that communication doesn't always require exact words. Currently mentored through the EMDR certification process by Kathy Couch at EMDR University, Messina is committed to expanding access to neurodivergent-affirming therapy and helping clients discover their sense of self beneath the information overload. She believes deeply in the power of play, curiosity, and creating spaces where hard questions are finally welcome. When she's not in session, Messina can be found observing the world around her, managing her relationship with the Texas sun, and advocating for better understanding of neurodivergence in clinical practice. In this episode, we dive into: • Trust your client always - If a neurodivergent client says "I can't feel it," believe them. Don't assume it's trauma-related or tell them they're "doing EMDR wrong." • Create sensory-safe environments - Turn off overhead lighting, neutralize scents, minimize distractions, and dress consistently to support regulation and predictability. • Getting to zero isn't always the goal - For neurodivergent clients, focus on state change and adaptive functioning rather than insisting on a SUDs rating of zero. Confidence in self matters more than the number. • Neurodivergence + trauma requires nuanced assessment - Symptoms can be misdiagnosed as BPD, bipolar, or depression when neurodivergence and trauma are actually at play. • Use visual tools and creative approaches - Incorporate art, Mr. Sketch markers, written words in color-coded formats, and allow movement during bilateral stimulation. • Play and attunement are essential - Behavior is communication. Build rapport through unsaid understanding and let clients process in their own way. Psychotherapy became her second career following many years in business, although her interest in trauma began in central London in her 20’s, where she volunteered at a helpline and drop-in center for the suicidal and despairing. She became a certified EMDR therapist in 2010 and honed her expertise in the healing of trauma with the addition of Thought Field Therapy tapping techniques and energy psychology to her work. Over the years she has worked with thousands of clients with attachment and developmental trauma, physical, sexual and emotional trauma, medical and childbirth trauma, and those who have experienced accidents, injuries, scary events and other traumatic losses. She is known for being able to precisely trace the past roots of traumatic symptoms showing up in the present, and by carefully combining several trauma-healing modalities, is able to provide efficient and effective release without the emotional upheaval that often accompanies deep trauma-work. For ten years Jane taught graduate students in Bethel Seminary’s Marriage & Family Therapy program as a Faculty Associate, and was the department’s expert on trauma and its treatment. In 2017, Jane certified as a coach to extend her reach across state and international lines, and now trains coaches internationally on becoming trauma-informed. More recently she became certified as an advanced practitioner in Akashic healing, bringing an extra bit of spiritual assistance and magic to her interventions. She works via video with clients from all over the world, healing trauma, encouraging love for ourselves, and helping all of us access the magic of our being. Ms. Pearce currently facilities a private practice in the Pocono, PA area that customizes services for individuals & couples impacted by grief & loss across the life span, maternal mood disorders, reproductive trauma, developmental trauma & infertility.
