In more than a decade of training and clinical practice as a psychiatrist, I've become increasingly disillusioned by the American healthcare system. It prioritizes profit over progress at the expense of OUR health. The average American spends almost twice as much on healthcare as the next highest spending country, yet our life expectancy drags four years behind other industrialized nations and ranks OUTSIDE the top 50 in the world. The system is a disaster, yet it's broadcast as 'the greatest healthcare system in the world.' Renegade Psych wants to return medicine's focus to PROGRESS OVER PROFIT, and limit the influence and power of the pharmaceutical industry and big business entities, whose primary motivations are financial in nature.This conversational podcast is released monthly via SERIES on particular topics. There are 8-12 SEGMENTS, or episodes, per topic. I interview internationally-recognized experts in their fields AND relatively unknown, up-and-coming healthcare provi

49.1 Brazil perspective: Immoral Marketing of SSRIs, MAOIs, Lithium, Benzo's - FULL Interview Antonio Nardi, MD
Hey folks, I'm trying something different by posting my full audio episodes on all non-YouTube platforms monthly instead of weekly. This conversation is with Antonio Nardi, MD, and we discuss the immoral and aggressive marketing of SSRIs, overpromised in terms of efficacy and safety profiles, which led to the near-dissolution of use of the MAOI class of antidepressants, which are significantly more effective than SSRIs in treating depression. We also talk about the underutilization of lithium, and how to appropriately use benzodiazepines in clinical practice and find the balance between creating dependence/withdrawal cycles AND using them when they're indicated for short-term or intermittent use. What I see in clinical practitioners is that older docs/providers UNDERESTIMATE the risks of addiction and younger professionals tend to OVERESTIMATE the risks and UNDERUTILIZE this very effective (in the short-term) tool of psychopharmacology. 37.2 ADHD is Overdiagnosed/Treated, DSM is invalid with David Cohen, PhD, MSW

In Part 2, David and I discuss the over-diagnosis of ADHD and rapid inflation of diagnosis and medication use over the last 20-30 years (the US utilizes 90+% of the world's amphetamines, and the vast majority of all pharmaceuticals treating ADHD compared to the rest of the world), as well as the invalidity of the DSM, or Diagnostic and Statistical Manual of Mental Disorders, which is notably NOT AT ALL statistical, and mongers up diagnoses that aren't unique from other mental health diagnoses. This is a common trick used by big pharma to push an idea about a diagnosis in order to lay the foundation for their drug that is utilized to 'treat' the diagnosis (which, let's be real, usually ends up as a treatment of a couple of the symptoms of the diagnosis and then they call it 'statistically significant' of an effect and obtain FDA approval. 35.2 Eugenics in Psych's Origins Part 2 + Steve Hayes, PhD
Here, I wrap up with Part 2 of my talk with Dr. Steven C. Hayes, PhD, about the embedded history of eugenics in psychiatry and psychology, way before Nazi-era Germany's Holocaust atrocities. After talking about the history of eugenics in bio-statistics and psychology/psychiatry's origins and legalized sterilization of those deemed 'feeble-minded,' in Part 2, we expand on how we must take precautions to prevent these types of atrocities to occur in a modernized way today. Steve tells us about better statistical ways of measuring individuals over time, then combining millions of individuals over billions of years-lived to create both population-wide predictive models of behavior, but also individualize our treatments by allowing AI to help us learn which processes (acceptance, defusion, contact with the present moment aka meditation/mindfulness, self-as-context, values, and committed action/goals). We can't let government or corporate interests to dictate what constitutes good science and medicine, and need to re-distance ourselves from those entities, with government playing an ACTUAL REGULATORY role and business driving safe and effective innovations in medicine. 35.1 Eugenics in Psych Origins with Steve Hayes, PhD
Today, I talk to Dr. Steven C. Hayes, PhD, about the embedded history of eugenics in psychiatry and psychology, way before Nazi-era Germany's Holocaust atrocities. We discuss the history of eugenics in creating and adhering to some of the fundamental tools in statistics, and how that paved the way for many of the original leaders in psychology and psychiatry (late 1800s, early 1900s) to be prominent eugenicists. Legalizing mass sterilizations of those deemed 'feeble-minded' in the US was backed by THE SCIENCE of the early 1900s and predated Nazi Germany by a couple of decades. While this episode is pretty disturbing for those not in the know, it is really important for us to understand our history so we don't repeat the same horrendous mistakes and fall into similar unscientific patterns moving forward. Steve implores us to expand our statistical methods and repertoire in an effort to prioritize measuring change in individuals over time as a way to tailor treatment to each individual. During medical school, Jill was diagnosed with an aggressive form of breast cancer, and fought through treatment while continuing her medical school journey. Treatment was effective, but led to severe side effects/consequences in the form of Crohn's Disease, a type of inflammatory bowel disease that causes digestive tract inflammation and sometimes requiring major abdominal surgery to resect diseased parts of the bowel. Through all of this, Dr. Carnahan persevered, and used her experiences to improve her knowledge and understanding of what patient's need in her blend of holistic and Western medicine approaches. She very much takes a preventive and root-cause approach to her patient's care, as opposed to just identifying and treating symptoms. She recognizes the role of inflammation in our rising rates of chronic disease and mental illness and works to reduce inflammation in a flexible way with her patients, offering a multitude of potential treatment options. She utilizes functional labs to target direct/root causes of illness and, like me, is a big proponent of utilizing nutritional and low doses of lithium to improve mental and physical health outcomes. He outlines how the serotonin system is extremely important for fetal brain development, with animal studies showing obvious changes in brain development for fetuses exposed to SSRIs and now human studies showing distinct brain changes for adolescents (based on long-term follow up MRI scans) who were exposed to SSRIs during their mother's pregnancies. Dr. Urato argues that based on the available data and research, SSRIs SHOULD BE CATEGORY D IN PREGNANCY, instead of their current status as Category C, indicating animal data has shown potential toxic effects, but human data has yet to reveal the same. Dr. Urato and I are both adamant that this is not a form of pill-shaming, but rather an effort to educate people about the true effects of these drugs in pregnancy as opposed to only receiving the pharmaceutical-sponsored information on this topic. 31.1 Functional Psychiatry with James Greenblatt, MD
Join me as I sit down with Dr. James Greenblatt, MD, to discuss his work within the field of what's called functional, or integrative psychiatry. Dr. Greenblatt uses many of the tools he learned in medical school paired with an integrative critical thinking approach. He tests for mineral and vitamin deficiencies, indirect markers of neurotransmitter abnormalities, AND ensures every patient gets a full medical workup to rule out medical causes of depression, anxiety, ADHD, autism, bipolar, schizophrenia, and many more psychiatric diagnoses. Dr. Greenblatt is anti-DSM due to its' primary focus on subjective symptom reports, which oftentimes substantially overlap between so-called 'distinct' psychiatric disorders, and instead tries to gather as much objective information and data as possible in order to help treat his patients. We talk about the role of, not just high doses of lithium in bipolar disorder but also, low doses of lithium for irritability, impulsivity, and cognitive rigidity, with underrecognized (and sometimes incredible) results in ADHD and autism, as well as any suicidal patient. While some view Dr. Greenblatt's approach as 'quackery,' I'm here to tell you I've started to test for some of the same things and am finding abnormalities in lab work that guide my treatment, and now I'm starting to see results in my patients. It's so much more exciting than the machine-gun-full-of-SSRIs traditional approach to practicing psychiatry, where 75% of the algorithms dictate that I prescribe an SSRI for everything from depression to anxiety to autism to post partum symptoms to eating disorders and OCD, etc. etc. Practicing 'idiot' medicine is not why I went into medicine and Dr. Greenblatt offers a unique and holistic approach that combines techniques of traditional psychiatry with this new wave of a more functional approach. 30.2 Vaccine Nuance, THE SCIENCE, and Alzheimer's Drugs with Nikki Johnson, MD

Neither of us are for or against ALL vaccines (and both of us have fully vaccinated kids... well, not including COVID or flu...), but rather take them in piecemeal and openly discuss the need for more research on their long-term safety and efficacy. We also delve into our appreciation of RFK Jr.'s attempts to clean up our food (and the skepticism that removing things like pesticides will be all good without any downside), then air our grievances with his lack of clarity around important societal conversations like which vaccinations he universally recommends (and the dangers of re-emergence of things like measles outbreaks due to poor vaccine uptake). Nikki brings up Tracy Beth Hoeg, a Danish-American scientist and physician who has done deep dives into the differences between Denmark's vaccinations schedule and ours in the United States. Additionally, Dr. Johnson compares what big pharma is doing with the new class of drugs to treat Alzheimer's to the Tuskegee syphilis experiment, in the sense that people are unknowingly being experimented on with a class of drugs that does not have anything close to a favorable benefit : risk ratio. We also delve into our appreciation of RFK Jr., as well as our beef with his lack of clarity around important societal conversations like which vaccinations. We also talk about how and where the anti-psychiatry movement goes too far, emphasizing that some individuals DO NEED biologic treatments, but big pharma preys on medical school curriculums, trying to push the narrative that so many more individuals NEED biologic/medication treatment, when there are so many effective and cheap natural remedies for so many of our problems. We openly discuss the flawed SSRI antidepressants, and reference their many failed, or negative, trials, as well as the manipulation of data pervasive across several SSRI makers clinical trials. Roger and I are tired of big pharmaceutical interests and their government regulators blatantly LYING to us about the safety and effectiveness of the treatments they promote, so we decided to have a conversation about it! 27.2 Misdiagnosed and Overmedicated Kids with Kassondra Ola, Part 2
Today, we have Kassondra Ola joining the podcast for part 2 of a patient perspective interview on the dangers of psychiatric polypharmacy. Kassondra is a mental health advocate who was misdiagnosed as bipolar at the age of 10 and put on multiple psychiatric medications over the subsequent decade. She is here to tell her story and promote a positive message that we, as psychiatric prescribers, need to think long and hard about whether or not to use medications in youths and teenagers... and if we decide medication is necessary or warranted, to do it extremely carefully, monitoring for immediate and sustained progress of each medication, giving time for medications to have an effect, and tapering patients off of one medicine before we start the next medicine. This way, kids don't end up in the position Kassie and so many other children end up in being prescribed several medications at once and having unmanageable side effects. In 1987, we medicated 0.2% of our youth with mental health problems and prominent psychiatrists of the time believed severe mental illnesses essentially required puberty and the resultant expansion of consciousness and brain development. Now, we medicate 15-20% of American youths... Have we really become so mentally ill in a matter of 2 generations, OR are we being marketed and sold pharmaceutical products as if they were child play things... you know where I stand on that question, but what do you think?
