Pilot Ladders - Do or Don't
Providing a legally compliant pilot ladder in accordance with the latest International Maritime Organization (IMO) requirements, particularly the rigorous standards set forth in SOLAS Chapter V, Regulation 23 and recently reinforced by the 2025 adoptions of Resolutions MSC.572(110) and MSC.576(110), is absolutely critical for safeguarding the lives of maritime professionals during one of the most inherently hazardous operations at sea. The physical transfer between a small, rapidly pitching pilot boat and a massive, moving commercial vessel exposes pilots to severe risks—including fatal falls, crushing hazards, and drowning—which are drastically magnified if the ladder features defective construction, improper rigging, or non-compliant securing methods such as unauthorized deck shackles or degraded side ropes. Box Boat Blues
Parametric rolling becomes exceptionally hazardous in shallow water because the vessel's vertical motion is constrained, and the interaction between the hull and the seabed significantly alters the ship's stability profile. When an ultra-large container ship encounters waves in shallow regions—such as the North Sea or the approaches to major ports—the "squat effect" increases its effective draft, while the restricted water flow beneath the hull can lead to more abrupt and violent stability fluctuations. As wave crests and troughs pass along the ship, the rhythmic changes in the waterplane area are amplified by the shallow-water wave dynamics, triggering rapid roll angles that can exceed 30 degrees if unchecked.Barry shares his personal experience of this effect, relates it to real world piloting limitations and discusses the case of the MSC Zoe from 2019. When Case Law Blurs Safety.
The collision between the bulk carrier Polesie and the general cargo ship Verity on October 24, 2023, was caused by a failure of both bridge teams to effectively apply the 1972 COLREGs in a crossing situation. Verity, as the give-way vessel, failed to take early and substantial action to stay clear, while Polesie, the stand-on vessel, did not maintain a predictable course and speed, instead making small, incremental course adjustments to port that compounded the confusion. Contributing factors included a lack of direct bridge-to-bridge communication—exacerbated by the VTS radio configuration—and the absence of a Voyage Data Recorder on Verity, which hindered a full reconstruction of the final decision-making process. Barry discusses the new report and highlights areas where he feels the report did not discuss fully enough. The Prestige - Accident or Crime?
The loss of the Prestige remains one of the most contentious maritime disasters in history due to the fundamental conflict between the Bahamas Maritime Authority’s technical focus on structural failure and the Spanish authorities' emphasis on criminal negligence. While the BMA and international salvors argue that the ship was a victim of a "death march"—where the refusal of a Place of Refuge and the decision to tow the vessel into a North Atlantic storm caused its final breakup—the Spanish Supreme Court ultimately ruled that the Master and owners acted with "environmental recklessness." The Oil Compensation Minefield.
While the CLC 1992 and the 2001 Bunker Convention are both strict liability regimes requiring compulsory insurance and allowing direct action against insurers, they are mutually exclusive in scope: the CLC 1992 focuses specifically on pollution from tankers carrying persistent oil as cargo, whereas the Bunker Convention fills the "gap" by covering fuel oil spills from non-tanker vessels (such as bulkers and container ships). A critical distinction lies in their structure, as the CLC provides a specialised multi-tier compensation system linked to the IOPC Fund with its own internal liability limits, while the Bunker Convention lacks an international fund and relies on separate regimes like the LLMC for limitation of liability, making liability under the bunker convention much smaller.Barry discusses both funds and explains why owners may try and apply the bunker convention over the 1992 CLC to limit their liability. Which Fuel at the Pump?
While both Marine Gas Oil (MGO) and Ultra Low Sulphur Fuel Oil (ULSFO) both meet the 0.1% sulphur limit for Emission Control Areas (ECAs), they are fundamentally different: Marine Gas Oil (MGO) is a pure, "clear and bright" distillate that behaves like high-grade diesel and is generally the most reliable and expensive option, requiring no heating but posing risks of low-viscosity leakage or "dry" injector wear. In contrast, Ultra Low Sulphur Fuel Oil (ULSFO) is typically a "hybrid" blend containing residual oil components, making it cheaper and more lubricious but technically complex, as it requires onboard heating to reach injection viscosity and carries a high risk of chemical instability or "sludging" if mixed with other fuels.Barry discusses a recent act of pilotage that he did where the choice between the two types of fuel may have made the main engine fluctuate and the Master forced to declare that he may have a problem. Cargo Overboard
On December 6th 2025, the Baltic Klipper lost 16 containers overboard in the Solent near Nab Tower after experiencing severe rolling in heavy weather. HM Coastguard was alerted and issued navigational warnings as the drifting containers posed a significant hazard to one of the UK’s busiest waterways. Aerial searches located several containers, and over the following days eleven washed ashore along the West Sussex coastline, including Selsey and Pagham. Although most boxes contained fruit and were not hazardous, authorities warned the public to avoid the wreck material. To Be or Not To Be a Narrow Channel
A narrow channel has no formal dimensional definition in COLREGs or IMO conventions; instead, courts, pilots, VTS authorities and the MCA determine it by practical navigational constraint. A waterway is considered a narrow channel when vessels must follow the dredged or charted track for safe navigation, with limited room for meeting, overtaking or deviation. Case Law reaffirms that narrowness is a question of fact, based on depth, width, hydrodynamics and vessel operational constraints. Strap On Another Tug!
Tugs play a critical role in maintaining control of large vessels during bad weather, when wind, swell, and tidal forces quickly overwhelm a ship’s own manoeuvring ability. High-sided vessels such as container ships, RoRos and cruise ships present huge windage areas, causing the bow or stern to fall off rapidly in crosswinds. At low speeds, rudders and bow thrusters lose effectiveness, leaving pilots with limited means to counter drift, yaw or surge. In these conditions, tugs provide the controlled, directional force required to keep the vessel on track, stabilise heading, prevent unwanted movement and maintain safe clearance from berths, dolphins and other structures.Beyond simple pushing or pulling, modern ASD, tractor and rotor tugs are essential safety assets in exposed approaches, river entrances and terminals where strong weather threatens to overpower the ship. They deliver high bollard pull, indirect escort forces and rapid corrective thrust that prevent groundings, allisions and loss of control. In heavy weather, tugs are not just helpful but indispensable—acting as the primary control mechanism that ensures ships can berth, unberth, turn and transit safely despite adverse conditions. A Chat with Sailing Legend Sir Robin Knox Johnson
Sir Robin Knox-Johnston’s lifelong relationship with the sea began early. Educated at Berkhamsted School, he joined the Merchant Navy at the age of 17, serving as a deck officer with the British India Steam Navigation Company. His early years at sea gave him both the technical skills and seamanship that would later define his legendary status. In 1968, Knox-Johnston entered the Sunday Times Golden Globe Race — the first ever non-stop, single-handed, round-the-world yacht race. Sailing his 32-foot ketch Suhaili, built by hand in India, he faced enormous isolation, equipment failures, and brutal weather. After 312 days alone at sea, he returned to Falmouth on 22 April 1969, becoming the first person in history to sail single-handed and non-stop around the world. Of the nine competitors who set out, only Knox-Johnston finished. He donated his £5,000 prize money to the family of fellow competitor Donald Crowhurst, who had perished during the race.This episode marks my 40th anniversary of being at sea, and I can't think of a better way to celebrate it then to talk with the man whose feat inspired me to not only take up sailing but eventually to go to sea myself. IMO Net Zero - A Passionate Ambassador John Taukave
The extraordinary session of the IMO's Marine Environmental Protection Committee (MEPC) session was convened on the 14th October to formally adopt the NZF as amendments to MARPOL Annex VI, which would introduce for large oceangoing ships a global fuel-intensity standard and a GHG emissions-pricing mechanism. However, member States were unable to reach consensus and therefore did not formally adopt the framework at this meeting. Instead they voted (57 in favour, 49 against/abstain) to adjourn the decision for one year and reconvene in October 2026. This was a disappointing outcome for the IMO. Three days later I interviewed John Taukave who was present on this committee. This podcast showcases that interview. Williams Shipping - The Wildiscover - 131 Years in the Making.
In 1894 George Williams gave up his career working on windjammers to set up his own business. Aged 30 George acquired a small pinnace which was being sold by the Royal Navy. He rigged a sail and brought the vessel to Hythe on the banks of Southampton Water. He set to work and converted the boat into a small sailing barge which was able to carry up to twenty tons of cargo. It was not long before he was successful in acquiring his first cargo collecting a consignment from the Isle of Wight for delivery to Southampton.Today, 131 Years later, Williams Shipping still trades strongly around the UK Coast and is still based in the port of the company's birth - Southampton. The trend in seafarer abandonment is clearly escalating. From an already catastrophic record in 2024, the situation continues to deteriorate in 2025—both in scale and severity. Key regions, flags of convenience, and weak enforcement remain persistent contributors to the crisis. 2024 marked a grim milestone, becoming the worst year on record for seafarer abandonment:3,133 seafarers were abandoned onboard 312 vessels, an 87% increase from 1,676 seafarers and 132 vessels in 2023. A bow thruster is a transversal propulsion device installed in the forward (bow) section of a ship, allowing it to move laterally (sideways) without forward motion. It is especially valuable for low-speed manoeuvres such as berthing, unberthing, or holding position in dynamic positioning (DP) systems. While it greatly aids ship handling, it comes with several limitations.The bow thruster was first developed in the mid-20th century to improve low-speed manoeuvrability in increasingly larger ships. Early versions appeared in the 1950s and 1960s, particularly on Scandinavian ferries, using simple mechanically driven side propellers. By the 1970s, tunnel-type bow thrusters became standard on tankers, ferries, and cruise ships, often powered by diesel or electric motors. Transverse Thrust - Friend or Foe...?
A ship’s propeller is a marvel of marine engineering — a seemingly simple rotating device that plays a vital role in moving massive vessels across oceans. At its core, a ship's propeller converts rotational power from the ship’s engine into thrust, propelling the vessel forward or backward through water. A ship’s propeller works by spinning its blades to accelerate water in one direction, thereby pushing the ship in the opposite direction. Though it may appear simple, its function is governed by sophisticated physics and engineering. Transverse thrust (also known as side thrust, asymmetric thrust and paddle wheel effect) is a phenomenon associated with the rotation of ship propellers, particularly noticeable when a ship is moving at low speeds or starting from rest. Lookout! You're Breaking the Rules
On March 10, 2025, a significant maritime collision occurred in the North Sea, approximately 12 miles off the coast of East Yorkshire, England. The Portuguese-flagged container ship MV Solong collided with the US-flagged oil tanker MV Stena Immaculate, which was anchored at the time. Electric Shock or Enclosed Space - both are lethal!
Enclosed space accidents on ships typically involve asphyxiation, toxic gas exposure, or falls due to lack of oxygen. These incidents occur in spaces like cargo holds, ballast tanks, fuel tanks, pump rooms, and void spaces. However a recent report by the MAIB into the electrocution of a chief engineer on a super-yacht highlights some concerning issues.Was the poor fellow electrocuted or had he collapsed onto the terminals for some other reason. The actual sequence of events will never be fully known, but the report highlights some rarely documented risks. Coming into Effect in 5 days! Marine Pollution Special Area - The Red Sea
Whilst the Red Sea was designated a special area under the International Convention for Preventing Pollution from Ships (MARPOL) and was adopted as such in 1973, the restrictions on the discharge of oil were yet to come into effect. Although signatory states were required to enforce the restrictions, they do not come into effect until 1st January 2025. MLC 2024 Ammendments
The Maritime Labour Convention came into force in August 2013. Since then there have been subtle but accumulating changes to the original text which have sought to enhance the rights of seafarers. 2024 sees more changes coming into effect from the 23rd December including the provision of internet. In a world of connectivity this basic right has so far been overlooked. But in recognition of the improvement to mental health that social connectivity has, it now forms part of the recent changes. Red Sea Tragedy - A Bad Trend Indeed.
The 'Sea Story' was a 'large yacht' style ship that acted as a base for dive activities in the Red Sea. It had cabins onboard for 36 guests and 18 crew. It tragically capsized off the Egyptian Coast on the 25th November 2024. She capsized suddenly in heavy weather with the loss of at least four lives.She wasn't the only super yacht to do this unexpectedly in 2024. The Baysian was another very high profile tragedy that struck when freak weather caught out an unsuspecting ship.Being in 'Port Mode' with regards to the watertight integrity when not in a sheltered port is a worrying trend on this size of ship.
