The Badwater virtual 267mile race is almost completed and I feel great running an average of 16.7 miles a day on a mere 4 grams of carbohydrate. I would never of guessed, but I am quite happy about it. Today I will review in a much different view, The China Study. It was first published in 2006 and I have read and referenced it several times to justify my plant based dairy free diet. I read the book but did not dig deeper until now. I will share the concerns with you on the episode. It is enough to dismiss all of the findings? No. What good came out is that we should eat real food. No processed. Avoid sugar and seed oils. However, the protection from plant proteins are certainly suspect versus animal protein. Listen, learn, read and make your own conclusions. Welcome! and Thank you for listening. This episode is about early data of my personal continous glucose monitor. These sensors can be purchased without a prescription and are used to monitor the glucose in the interstitial fluid. Translated there is a mild delay from the blood to the cell fluids where the glucose is monitored and then uploaded to an App on your smartphone. Diabetes and glucose intolerance are the biggest risk factors for cardiovascular disease. Often, people assume that their if glucose is normal fasting, then they are ok. The problem is that insulin resistence occurs much sooner and still carries the risk of chronic insulin elevations. Welcome! and Thank you for listening. This episode will highlight my latest ultra at Croom's Fool run where I completed 35 miles/50K. The original goal was 50 miles but the day went a little different. Nevertheless, I finished feeling well and continue to learn about low carbohydrate fueling and ultra distance events. I will also share a story of doubt that I observed. The story we tell ourselves can seem benign and even humble, but it is sometimes setting us up for failure. We have 70,000 thoughts per day and about 80% are negative according to research. 95% are repetitive. That means we keep telling ourselves negative things quite often. That would easily be labeled as "bad parenting" today but it seems perfectly ok to subject ourselves to it. I see similar things in the medical community. Providers are not willing to see challenging information as learning or keep an open mind to change. The "practice of medicine" is just that. Practice and life long learning. We are not inferior or wrong if we change our minds. It is evolution of learning. Welcome! and Thank you for listening. Today's episode is my story of making a quit for my granddaughter , and how it was not in my comfort zone. I learned to quilt from watching my grandmother. Unfortunately, that was 50 plus years ago. She made quilts for her family by hand with fabric from old clothes. She allowed me to quilt with her using black thread so it could be easily seen and removed when I went home at night. I remember the care and hard work that went into making the quilts and how they warm and cozy they felt on a cold night. Quilting is piecing together fabrics from various sources. The fabrics are cut into patterns and sewn together. They are then attached to a batting layer that provides depth and warmth. The third layer is a backing. The quilting process is actually the stitching together of the three layers in various patterns depending on the complexity. You can sew by hand or with a machine or a combination. I did mine with a standard sewing machine. I have no training, just memories and video tutorials. It is the memories that are most important. The gift of time poured into a project to share with someone else. It lasts. It requires mindfulness and dedication to detail. It is art shared and is raw. To me it is a symbol of life weaved piece by piece. Some of it smooth, other parts uneven and challenging. I hope it is a sense of peace and love that is passed from one person to another. Welcome! and Thank you for listening. Don't fall off your exercise ball but we are going to talk low carb and keto! Last week I completed the Long Play 33 with 35 miles- bonus miles as Andy, the race director, would say. Based on a recently published study on nutrition during endurance events, I decided to decrease my glucose intake and add Ketone IQ shots to the mix. I have a history of bonking in the later parts of a race. Sometimes, it is more like the early part. My stomach no longer accepts gels, I feel sluggish, and have assumed it was my mental attitude and the need for more fuel. I figured I was dehydrated leading to a decrease in blood to the gut and inability to digest my intake resulting muscle glycogen depletion. As soon as I started taking gels, I became dependent on them. Once, I could not tolerate them, I was headed for low energy and the need to walk or quit. My new hypothesis is that it is my brain's demand for fuel and my bloodstream dip in glucose that causes the bonk. Wild swings in glucose and insulin followed by a decrease in my ability to burn fat as fuel. So I tried a few new things this race and had a pleasant surprise. Welcome! and Thank you for listening. Long Haul Ultra marathon is a 100 mile race at Colt Creek State Park in Florida. It consists of 6 loops of 16.8 miles. The runner comes back to the start at each of the loop were there is an open aid station or individual crew access. Out on the course, there are 3 other times where there are aid stations for snacks and fluids. The first loop went well but things deteriorated. What happens when the best laid plans fail? This is the story of Long Haul. Why do we have to learn the same lessons with a little twist over and over? Diet and nutrition all sound so simple on paper. Eat a plant based diet and all will be well. That works until the questions arise and you find yourself confused with less than perfect options. One way out is to give up the fight, the other is to learn and grow from mistakes I will share my frustrations, disappointment and growth from this experience. I believe the learning points can be applied to any new task or long term goal. 1. Break a goal down into small parts. This seems obvious but the trick is staying in the small part and not looking into the daunting future. 2. Have a strategy for each segment before the segment begins and follow the plan. Variables come up, there is always the unknown, but a strategy is a must for a baseline. 3. What is your time frame? Can you stick to it? Have you ever started a trip without a time plan? The progress is not always what you want. 4. Stiffness is a real concern with sitting during an ultramarathon. However, you can get started again. 5. Nutrition is best handled without relying on an aid station for solid foods for the most part. Its is not a buffet. 6. Salty britches is a must for blister prevention. I hope you enjoy my story. It was a very special moment to have my grandchildren on my lap during this podcast. A simple conversation unscripted after a simple dinner of bean soup and cornbread. Colt Creek 45K was also a simple gift of running in the woods in central Florida. Even though our leaves don't change, there were beautiful purple and yellow wild flowers among fields of grass, pines and oaks. Welcome! and Thank you for listening. There is shocking news with this podcast that I hope will allow all of us to become healthier. This episode I will discuss coaching and being coached. We are all always learning. During our time in formal education situations, it is assumed that the teacher knows best. Do as they say, get a good grade, and proceed in achieving your goals. We trust that they will provide accurate information to the best of their abilities. It is expected that one completes the assignments to the best of their abilities. Grades are assigned for performance. But we learned in a stepwise fashion. Simple addition before calculus. Our exceptions were based on that progression. Some of us developed a need to please or get good grades. The reward being good grades, good job, and self esteem. As adults, we are still leaning at our own pace. However, the information is infinite and abundant. Only the individual knows the level of their current knowledge. The coach has a singular goal to get the best results possible for a wide range of students. We want to please the coach. As adults, we can ask questions, but we still want to please the coach. This applies to any learning situation. My job as a physician is to educate my patient-students. They come from a wide variety of backgrounds and interests. I also have a coach for ultra running. None of us want to come up short in the eyes of the teacher. The ultimate goal is good communication that allows everyone to learn from their own starting point. That is what my practice has always been about. Our success depends upon accurate feedback and adjustment from both the teacher and the student. DHA is an omega 3 fatty acid that is abundant in the brain, the eye, and around the covering of nerves. It is found in marine animals and algae. Small amounts can be converted from plant sources of ALA like flaxseeds, hemp seeds, and chia seeds. DHA levels have been found protective for mental health, cardiovascular health and immune health. Have you passed the DHA test? I have found that just consuming ALA is not adequate for good DHA levels in the majority of vegans. That means some form of supplementation is necessary. I will discuss this in this episode. The people are wonderful and it is a beautiful little town with lots of trails for mountain biking, running and hiking. There are also lots of rocks. I have been plant based nearly 25 years. I eat fruits and vegetable as close to their natural state as possible most of the time. I do not sell supplement and believe the best way to get nutrients is from the food. We do need a B12 supplement. I have spoken about the importance of Vitamin D from the sun but the potential need for supplementation. That said eating a nutrient complete plant based diet takes some focus, and recently I have discovered some deficiencies that could easily be missed. Just like stress, distractions never go away, we just have the opportunity to react differently. Distractions occur both externally and internally. Internal distractions fuel the fire with the stories we tell ourselves to justify behaviors not inline with our goals. That internal dialog can be quite convincing. Welcome! and Thank you for listening. Cruel Jewel was not the outcome that I was looking for with regard to finishing. The details are shared in this podcast. The questions came earlier. Am I too old, too slow to run these races? Why am I doing this to myself? At 48 hours post race it sank in. It is more about the start line than the finish line. It is about taking a chance on the unknown. I had the opportunity to confront a struggle that I chose which is different from having a struggle cast upon me. I was able to experience that moment when all that exists is now. I focused on the trail. I watched for snakes, turns, rocks and roots. I climbed for miles and pushed my limits. I practiced going down through roots and rocks. I heard birds, saw beautiful rocks and traversed lush green forests (some secondary to poison oak). I experience the struggles of others and the help and encouragement of strangers and friends. I felt immense peace in the struggles. I am better for having started. My finish line came earlier on Saturday, but I am anxious and grateful for the ability to find another start line and all the unknowns that it will bring. Each time I start, I learn and grow. Rarely is it perfection as defined by the outcome. It is not so different from life. We plan and rehearse, but the reality is that life is often much different. There are many hacks to lifespan on social media. GLP-1 Agonists such as semaglutide are all the rage for curing obesity, addiction and various other ailments. They run about $1600.00 per month. It is too soon to determine if they result in increased longevity but a simple sulfonylurea drug such as Glyburide at $14.00 per month! was studied to see if it could lengthen life in mice and it did! But there is a catch. There is also a catch with metformin another diabetic drug. Most people are very willing to take something that might help if it is cheap or free even if there are potential side effects. One thing that also improves longevity is increased muscle mass. And the best way to get big muscles is to lift heavy things on a regular basis. I will give you why I am even more motivated to lift heavy things and propel my body into the air on this podcast. Our mission is movement and nutrition to promote a healthy lifespan. Obtaining a balance plate of macronutrients include protein, carbohydrate and fat. What was the fork in the road for your change in health trajectory? My family history lead me to medicine, but it was a podcast on running that inspired me to change my practice. My first marathon was to raise money for the American Diabetes Association to "find a cure". It was changing my practice that lead to my practice success in treating diabetes. Running has been a positive influence on my physical and mental health. They say when you finish a marathon or an Ironman triathlon that you are changed. It is true, but the 100 mile ultramarathon took all of that up a notch. I will share a few stories about people in my practice that have had their own experience and others who continue to struggle. Jami Dulaney MD Plant Based Wellness Podcast Episode 504: You Don't Always Get What You Want But You May Get What You Need
Welcome! and Thank you for listening. I ran 33 miles out of 100 miles at the Long Haul 100 this past weekend and I am happy. Last year it would have been a major disappointment not finishing. This year I am grateful to have ran well and came away stronger than ever. I will share the nitty gritty details of the weekend in this episode. There were plant based vegan people running but the majority were not including the aid stations. That is my problem and not theirs. So we happily bring our own food and supplies for great tent living for 48 hours. We had 3 coolers and a jet burner to warm up food. Most of the food was made at home such as pasta salad, tu-no salad, mushroom burgers, seitan-tofu loaf, potato soup, and tomato soup. We had a fruit bowl and fruit juice as well as simple running foods of sunbutter and jelly and various bars. Being plant based did not decrease our fun or enjoyment hanging out with non plant based people at a state park. Living your best plant based life does not have to be difficult, restrictive or limited. When the goal is the healthiest lifespan your can achieve, nutrition and exercise are at the top of the list.
