Episode 43: Enhancing ACE Screening through HMG Care Coordination
First 5 Contra Costa is leading the local effort to implement Help Me Grow, a system of coordination used nationally to connect children to the early screening and community services they need to thrive. The HMG model has been implemented throughout the U.S. and in half of California’s counties. First 5 Contra Costa, the Contra Costa Crisis Center, and La Clinica de la Raza came together to develop a pilot to integrate ACE screening and response workflow utilizing the existing HMG model.

CommuniCare Health Centers strives to be humble and learn from their staff and the communities they serve, as well as commit to their own development in unlearning patriarchal, heterosexist, white-supremist systems, while learning anti-racism best practices in trauma informed care. Episode 25: The Workings and Exploration of a Clinic Garden and Outdoor Classroom
Last week was Food Champion Week at Aliados Health. We hosted a Food Champion Roundtable event, and taped two podcast episodes related to food and nutrition. This is the second podcast episode and features Edye Kuyper from CommuniCare Health Centers. Edye Kuyper manages CommuniCare Health Centers’ food programs that connect patients with healthy food and with each other in order to prevent and manage diet-related disease. Raised on a farm in Vermont, Edye served as a Peace Corps Volunteer in rural Mauritania. Edye holds an M.S. in International Agricultural Development from UC Davis. Prior roles include facilitating better integration of agriculture and nutrition development efforts in low-income countries, and providing technical assistance to community-based nutrition education and food security projects that engage low-income U.S. households. Edye also lectures in the Nutrition, Food and Dietetics program at CSU-Sacramento. CommuniCare Health Centers’ (CCHC) “Food is Medicine” initiatives integrate food with the delivery of clinical services. CCHC is located in Yolo County, where agricultural traditions run deep. Programs draw on unique community strengths to address patients’ mental and physical health needs while considering environmental justice. Initiatives to date include the Garden and Outdoor Classroom, cooking classes, and produce distribution and prescriptions.

Episode 24: Alameda County Food as Medicine Initiatives
Last week was Food Champion Week at Aliados Health. We hosted a Food Champion Roundtable event, and taped two podcast episodes related to food and nutrition. This is one of the podcast episodes and features Dr. Steven Chen (Chief Medical Officer) of All In Alameda County and Jessica Jamison, MPH (Director of Patient and Community Engagement) of Tiburcio Vasquez Health Center. They discussed their many food initiatives including food as medicine. ALL IN Alameda County expanded its Food as Medicine to Tiburcio Vasquez Health Center in February 2020. Tiburcio is a primary care clinic in San Leandro, serving the Ashland and Cherryland districts. This partnership builds upon the prior success of Food as Medicine at Hayward Wellness, part of Alameda Health System. Food as Medicine provides an integrative model for healthcare that addresses the social determinants of health and chronic disease. This model shifts medical practices by giving healthcare providers in Alameda County’s network of community-based health clinics the tools, training, and infrastructure they need to utilize healthy food interventions in combination with group behavioral support to improve health, socio-emotional wellbeing, and food security for Alameda County residents.

Episode 22: Expanding Food Access & Improving Nutrition Education Through a Food Pharmacy
This episode features Michaela Moss, a health educator from Marin City Health and Wellness Center (MCHW), who will talk about their food access and nutrition education initiatives. This is a unique episode, as it was recorded in March of 2020, just as the pandemic was starting to affect health centers. Many of the health center’s patient programs, including an in-person food pharmacy program, have been put on hold since the recording due to the pandemic.

Episode 21: The Power of Medically Tailored Meals
This episode features Cathryn Couch who is the founder and CEO of Ceres Community Project, a non-profit working to foster health by connecting people to one another and to a healthier food system. Ceres provides 150,000 organic medically tailored meals annually to primarily low-income people struggling because of a health challenge. Youth volunteers grow food and prepare the meals as part of a youth development, culinary and food system education program. Ceres has trained a dozen communities across the United States and in Denmark to replicate their model and the organization supports 8 Affiliate Programs. Couch is the 2020 chair for the California Food is Medicine Coalition, a six-agency collaboration conducting the first statewide medically tailored meal pilot for MediCal members. Ceres is also one of four agencies currently conducting large-scale randomized control trial for medically tailored meals at Kaiser Permanente. Couch is as advisor to the Aspen Institute’s Food & Society initiative; contributed to the Rockefeller Foundation’s 2020 report Rest the Table, sits on the leadership team for Hearts of Sonoma County – a collaborative working to reduce heart attacks and strokes; and participates in Marin Food Policy Council, California Food & Farming Network, and the Food Lab at Google. Episode 20: Patient-Centered Medical Home Transformation & Behavioral Health Integration
This episode features health center staff Michelle Hill and Avani Bedagkar. Michelle is the CQI Coordinator at Long Valley Health Center (LVHC) in Laytonville, CA. She was the Patient-Centered Medical Home (or PCMH) Project Lead when LVHC applied for certification in 2018 using the new 2017 Standards and Distinction programs. LVHC was the 1st Health Center in California to receive the Distinction for Behavioral Health Integration. Michelle will discuss the application and renewal process for the PCMH Distinction for Behavioral Health Integration. This distinction helps practices provide comprehensive whole-person care that acknowledges the behavioral health needs of the individual beyond the core requirements of NCQA PCMH Recognition. Avani is the QI Manager at Marin Community Clinics (MCC) and joined in 2017. She is the PCMH Manager for both Annual reporting and Transformation and works with other team members to ensure timely project completion. Avani will talk about the annual reporting process of PCMH. MCC is still going through the transformation process, so will talk about the basic structure of how to provide evidence for the transformation process.

In January of 2020, Sonoma Valley documented a promising practice titled “Continuous Glucose Monitoring for Uncontrolled Diabetic Patients”. While a lot has changed since January, Sonoma Valley has continued to expand its Continuous Glucose Monitoring (or CGM) project and are seeing significant results.

This episode is all about childcare. Childcare is an ever-present need for our workforce. Every county in California has a Resource and Referral (R & R) agency that serves as a hub for parents and childcare providers. They connect children and open spots, train providers, and manage financial assistance options that are available. This panel brings together the Executive Directors from the R & R agencies in Marin, Sonoma, Napa, and Solano counties to discuss concerns and potential solutions that they can help health centers to implement in both the short and long term. Episode 17: Patient Portals
This episode features Jennifer Inden, Aliados Health’s Health Information Technology Project Manager. She is joined by three health center representatives who will explore the different facets of realizing a successful Patient Portal. A Patient Portal is a secure website or app which patients can access certain health records, review lab results, and send messages to their care team. For years it has been acknowledged that the use of health information technologies and online resources has great potential to boost care quality and increase patient engagement in their care. Despite the advantages to both patients and providers, the adoption of patient portal use can be slow on both sides. The COVID-19 Public Health Emergency has shown once again how important it is to have a way for patients and providers to communicate with each other during non or limited business hours. This discussion includes Jane Brown, Operations Manager at Humboldt Independent Practice Association, Heidi Garzo, EMR Training Specialist at Petaluma and Rohnert Park Health Centers, and Summer Penn, Director of Information Technology at West County Health Centers.

Episode 16: COVID-19 Series – Aliados Health’s Response
This episode is the sixth in our series of COVID-19 focused sessions. The goal of these sessions is to share information with health centers that can help make this challenging time a little bit easier. This will be our last COVID focused session and features Aliados Health’s CEO Dr. Karen Milman who will talk about Aliados Health’s response to COVID-19. Dr. Milman has focused her career on improving systems to address the health of both individuals and the community. She is passionate about promoting health equity. Dr. Milman has over ten years of experience working in governmental public health, coordinating health improvement coalitions, and overseeing programs in chronic disease prevention, communicable disease control, environmental health, and emergency preparedness. She came to the Aliados Health from the California Department of Public Health, where she has been serving as a Public Health Medical Officer in the Emergency Preparedness Office. Prior to that, she was the Sonoma County Health Officer. Dr. Milman earned her medical degree from the University of Maryland and completed residency as well as obtained her Master of Public Health from Johns Hopkins University. She is board certified in Preventive Medicine and Public Health.

Today we have a continuation of our last episode “What Matters Most to You?” with Dr. Ellen Barnett. She is a recently retired family doctor who practiced for 20 years at the Integrative Medical Clinic in Santa Rosa and was on the faculty of the Sutter Family Medicine Residency from 1987 to 2016. She developed the Imagine You project as part of her work in the nonprofit Integrative Medical Clinic Foundation. Imagine You offers tools for self care that we can use for our own wellbeing and as we work with our patients and clients. In response to several recent disasters in Northern California, such as the wildfires, and now COVID-19, the Imagine You project offers trainings to organizations working with the most vulnerable in our communities. Our last episode introduced the “What Matters Most to You” tool, and focused on how to use the tool for self care. This episode will continue that conversation, going more in depth on the tool. Dr. Barnett will also share ways for health centers to use the tool in a healthcare setting with patients.

This episode is the first episode in a series of COVID-19 focused sessions. Our goal is to share information with health centers that can help make this challenging time a little bit easier. Aliados Health’s Health Information Technology Project Manager, Jennifer Inden, joined this session. She brings years of experience working in a health center, and at Aliados Health has become our resident expert on telehealth. In our current state, telehealth is the primary way health centers can continue to care for their high risk populations. If you’re finding it challenging to keep up with the ever changing rules and regulations around telehealth, you’re not alone. Throughout the episode Jennifer will be asked a series of questions to help clarify telehealth changes to date. She’ll provide some helpful tips around documentation and more.

Episode 9: Healthcare and The 2020 Census
This episode features Bethany Snyder, Aliados Health's ( Aliados Health) Advocacy Consultant, and Darlene Rios Drapkin, a Partnership Specialist at US Census Bureau. Our guests will talk about the upcoming 2020 Census. April 1 is Census Day! Every ten years the U.S. government conducts a census to count every resident in the US. It is a huge undertaking that greatly impacts the health care sector. Census data guides the distribution of more than $115 billion in federal funds to California each year. Aliados Health health centers serve over 230,500 patients each year, which amounts to millions of dollars in needed funding over the next 10 years. Our guests will discuss what you need to know about the census, how to reply, and how you and your health centers can ensure your patients, staff, and community get counted. Episode 7: Nutrition Counseling for Providers
This episode features Trina Higgins, a Registered Dietitian from the Northern California Center for Well-Being. A Registered Dietitian, or RD, is someone who is licensed to counsel patients on their dietary habits. To become a dietitian, one must obtain a four year degree with many specific dietetic course requirements, complete a dietetic internship, and pass a rigorous exam. More information on requirements can be found here. By 2024 it will be required that one earn a Master's Degree to be eligible to take the exam. In other words, there’s a lot of nutrition knowledge and expertise behind the title. While some health centers have an RD on staff, many do not. Those that do are often limited in how many patients that RD can fit into their schedule. We’ll save the issues around reimbursements for dietitians for another episode. Trina joined us to talk about nutrition counseling techniques for health care providers. This topic was requested by Aliados Health member health center providers. In general providers don’t receive much nutrition education and training in school, so we agree that some basic nutrition counseling techniques could help. Resources from Trina: The Harvard T.H. Chan School of Public Health's public-access website includes evidence-based nutrition information for providers who'd like basic nutrition education for themselves. The site includes information that applies to the general population, including their “Healthy Eating Plate” and other guidelines. American Association of Diabetes Educators and NovoNordisk Cornerstones4Care are resources geared toward diabetes management.

Episode 6: PHASE for Care Teams
This episode features Jessicca Moore, director of innovations at Petaluma Health Center who will talk about the PHASE program. PHASE stands for Preventing Heart Attacks and Strokes Every Day which is an evidence-based approach that first helped Kaiser Permanente reduce heart disease morbidity by 60 percent among its members. Aliados Health has participated in the PHASE program since 2011 with funding from the Kaiser Permanente Northern California Community Benefits Program and The Kaiser Foundation Hospital Fund for Community Benefit Programs at the East Bay Community Foundation. In 2020 Aliados Health will participate in an additional round of PHASE, with a new focus on health equity. Aliados Health participating health centers will measure and report on rates of A1c control among uninsured patients with diabetes. During this episode Jessicca will talk about how PHASE can be implemented as a care team and what roles members of the care team have to play.
