Why Sleep Deprivation Hits Harder When You Have T1D
SHOW NOTES:Sleep deprivation reduces insulin sensitivity in everyone. That's not a T1D-specific finding. Here's what is.In people without diabetes, the system has a feedback loop. Insulin sensitivity drops, blood sugar ticks up slightly, the pancreas compensates automatically, and the whole thing resolves before they're even awake. They make coffee, go about their day, and have no idea any of it happened. For T1D people, that feedback loop doesn't exist. The penalty just lands. This episode explains why the same sleep deprivation hits harder when you don't have a functioning pancreas to compensate -- and why T1D people have been quietly doing the backup system's job manually every single morning, often without realizing that's what they were doing. We're in Week 2 of the While You Were Sleeping Challenge. In this episode:How a healthy pancreas automatically compensates for sleep-related insulin sensitivity changesWhy the T1D body absorbs the full 21% impact without automatic correctionWhat "doing the backup system's job manually" actually looks like before the first cup of coffeeThe emotional reality of running on interrupted sleep while managing blood sugarWhat to add to your data tracking this weekThis Week's Challenge: On mornings after rough nights, notice if you had to work harder -- more corrections, more frustration, numbers that were less predictable. That's the 21% showing up in real life.Helpful resources and newsletter: https://yourbestt1dyear.comConnect with Neil: TikTok: https://tiktok.com/@the.betes Instagram: https://instagram.com/thebetes Facebook: https://facebook.com/neilgreathouse LinkedIn: https://linkedin.com/in/neil-greathouse-a607b912 Website: https://yourbestt1dyear.comBooks on Amazon: Type 1 Diabetes – One Day at a Time: https://a.co/d/6UHooWJ Type 1 Diabetes – True Stories: https://a.co/d/dfIlyI1

One Night of Poor Sleep Reduces Insulin Sensitivity
SHOW NOTES:Here's the number: 21%.One study. People with type 1 diabetes. Sleep-deprived condition (4 hours) versus adequate sleep (8.5 hours). Same food, same insulin, same activities. The sleep-deprived group showed a 21% reduction in insulin sensitivity the next day. Every single participant. This is the episode Neil has been building toward. If you've ever had a day where your insulin felt slow -- where corrections didn't land, where you corrected twice before breakfast and were still running higher than expected -- this episode gives you a name for what was happening. And it changes how you respond to those mornings going forward. This is Week 2 of the While You Were Sleeping Challenge. This is the research drop.In this episode:The specific study on sleep and insulin sensitivity in type 1 diabetes -- and what it actually showedWhat a 21% reduction in insulin sensitivity looks like in real life (the bathtub analogy)Why the effect showed up in every single participant, not just someWhat LeBron James figured out about sleep that the rest of us are just now learningHow to use this information to give yourself grace on mornings after rough nightsThis Week's Challenge: Look back at your data from Week 1. On the mornings after your worst nights of sleep, did your insulin feel different? Did corrections land differently?Helpful resources and newsletter: https://yourbestt1dyear.comConnect with Neil: TikTok: https://tiktok.com/@the.betes Instagram: https://instagram.com/thebetes Facebook: https://facebook.com/neilgreathouse LinkedIn: https://linkedin.com/in/neil-greathouse-a607b912 Website: https://yourbestt1dyear.comBooks on Amazon: Type 1 Diabetes – One Day at a Time: https://a.co/d/6UHooWJ Type 1 Diabetes – True Stories: https://a.co/d/dfIlyI1

What's Really Causing Your Unexplained Blood Sugars
SHOW NOTES:The pump site. The insulin. The food from six hours ago. The stress. The general vibe. Neil has blamed every single one of these for blood sugars that made no sense -- and for 34 years, he kept leaving one variable off the list entirely. This episode is about blood sugar archaeology: the 7am investigation T1D people run every morning. Holding the insulin vial up to the light. Going through the mental checklist. Sometimes coming up completely empty. The problem wasn't missing something obvious -- it was missing a variable that was never on the list in the first place. And on Monday, Neil delivers the specific number from the research that changes what that variable looks like.Week 2 of the While You Were Sleeping Challenge kicks off here. Today, he sets the stage and helps you look back at the data you've been collecting. In this episode:The full list of things Neil has blamed for blood sugars that didn't make sense (34 years of data)Why the T1D blood sugar checklist keeps coming up emptyThe "Law and Order at 7am" method of investigation -- and why it failsWhat the research says is the actual missing variableHow to look back at your Week 1 data before Monday's big episodeThis Week's Challenge: Think about the most confusing blood sugar you've had recently. Sleep and Blood Sugar in Type 1 Diabetes
SHOW NOTES:You went to bed at 112. Flat arrow. No active insulin. Textbook. You should be proud. You wake up at 218. So you run the checklist -- check the site, question the insulin, replay every meal from the day before. And you come up empty. Here's what this episode proposes: you were solving the right puzzle with the wrong set of clues.Neil connects sleep directly to blood sugar management in T1D -- not in a vague "stress affects glucose" way, but in a specific, directional way. The relationship between sleep and blood sugar runs both directions, and the second half of that equation is what T1D content has been leaving off the list. This is the episode where the puzzle starts to get a new piece. We're wrapping up Week 1 of the While You Were Sleeping Challenge. If you've been tracking your sleep and morning numbers, keep building. This is the episode that starts making sense of the data.In this episode:The two-way relationship between sleep and blood sugar in type 1 diabetesWhy morning blood sugars sometimes have nothing to do with what you ate the night beforeWhat happens to insulin response when sleep is disruptedWhy doing all the right things still produces confusing results sometimesWhat to add to your data tracking before next week's big numberThis Week's Challenge: Track sleep hours, morning blood sugar, AND how easy or hard it felt to manage your blood sugar the next day. Did your corrections land where you expected?Helpful resources and newsletter: https://yourbestt1dyear.comConnect with Neil: TikTok: https://tiktok.com/@the.betes Instagram: https://instagram.com/thebetes Facebook: https://facebook.com/neilgreathouse LinkedIn: https://linkedin.com/in/neil-greathouse-a607b912 Website: https://yourbestt1dyear.comBooks on Amazon: Type 1 Diabetes – One Day at a Time: https://a.co/d/6UHooWJ Type 1 Diabetes – True Stories: https://a.co/d/dfIlyI1

Why People with T1D Can't Sleep Through the Night
SHOW NOTES:When's the last time you actually slept through the night? No CGM alarms. No blood sugar math at 3am. No lying awake wondering if your number was stable. If you can't remember, that's exactly the point. In this episode, Neil digs into something that doesn't get said often enough: type 1 diabetes makes sleeping harder in specific, documented ways. Not in a general "I have a lot on my mind" way -- in a real, researched way that affects sleep architecture in T1D adults compared to those without the disease. From CGM alarms to actual lows to the anxiety of lying awake next to a flat-line CGM your brain still won't fully accept, this episode names what 3am with T1D actually feels like. We're in Week 1 of the While You Were Sleeping Challenge. If you missed Friday's episode, here's the whole thing in one sentence: we're tracking our sleep for eight weeks and figuring out what it's actually doing to our blood sugar. In this episode:What actually wakes T1D people up at night -- and why it's not always the alarmThe documented differences in sleep quality between T1D and non-T1D adultsWhy people with T1D spend more time in lighter sleep stagesWhat "T1D sleep mode" looks like (the nature documentary version)Why being awake at 3am with a stable number is not you being irrationalThis Week's Challenge: Write down what woke you up last night. One line. Alarm? Low? Anxiety? Just woke up? You're collecting data.Helpful resources and newsletter: https://yourbestt1dyear.comConnect with Neil: TikTok: https://tiktok.com/@the.betes Instagram: https://instagram.com/thebetes Facebook: https://facebook.com/neilgreathouse LinkedIn: https://linkedin.com/in/neil-greathouse-a607b912 Website: https://yourbestt1dyear.comBooks on Amazon: Type 1 Diabetes – One Day at a Time: https://a.co/d/6UHooWJ Type 1 Diabetes – True Stories: https://a.co/d/dfIlyI1

Why Your Mystery Blood Sugars Might Be a Sleep Problem
SHOW NOTES:If you've ever woken up to a blood sugar that makes no sense -- went to bed at a perfect number, did everything right, and still saw a confusing morning high -- this episode is for you.Neil Greathouse, who has lived with type 1 diabetes since February 14, 1992, kicks off the 8-week While You Were Sleeping Challenge by asking one question: what was happening the night before your worst blood sugar day last month? For 34 years, Neil blamed the pump site, the insulin, the food, the stress -- pretty much everything except the variable that was right there the whole time. This is the episode that puts sleep on the T1D checklist for good. This is Week 1 of the While You Were Sleeping Challenge. Whether you're starting here or catching up, you're not behind. Over 1,200 people are doing this challenge with you.In this episode:Why mystery blood sugars might have a cause you've never consideredThe full list of things Neil has personally blamed for blood sugars that didn't make sense (it's a long list)Why this 8-week sleep challenge is, in fact, diabetes contentHow to start the simplest data-collection habit of the whole challengeThis Week's Challenge: Write down how many hours you sleep each night. If you want to add one layer, note your fasting blood sugar the next morning. Don't change anything yet. The Mental Side of T1D that Nobody's Treating // Allison Nimlos, The Diabetic Therapist
79% of people with T1D experience shame related to their diabetes. Not occasionally. As part of their daily lived experience.Today's guest built an entire conference around that statistic. Allison Nimlos is a Licensed Marriage and Family Therapist (LMFT), a Certified Diabetes Care and Education Specialist (CDCES), and has been living with Type 1 Diabetes for 32 years. She knows this disease from the inside of a therapy room and from the inside of her own body.We talk about what diabetes distress actually looks like (and why it's not the same as burnout), why your A1C being good doesn't mean you're OK, and what it costs people to be stuck between an endo who doesn't ask how they're doing and a therapist who doesn't know what an A1C is.Plus, True or False, the Diabetes Confession Booth, and Allison goes head-to-head with the Fastest 5 Minutes of Your Life. The Diabetes + Mental Health Conference is May 1-2. That's next week. Virtual. $40 general admission. When Other People Offer to “Help Us” During A Low
Show NotesSuper Bowl Sunday is like a scientific experiment in chaos: long stretches of sitting, random grazing, adrenaline, and a snack table that basically yells your name. In this episode, we talk about what happens when a low hits in the middle of all that and why “I’ll just grab whatever’s closest” almost always ends with a 250 two hours later. We break down a simple three‑step game plan for game day: glucose first, snacks later on purpose, and one way to walk back to the couch without feeling like the “difficult diabetic friend.”You’ll hear how to separate “I’m low and need medicine” from “I want to enjoy the food at this party,” plus how to measure Super Bowl success in fewer roller‑coaster swings instead of a perfectly flat line. What You Can Do TodayDecide now what your “Super Bowl low plan” is: which glucose option you’ll use and how much you’ll take before you even look at the snack table.Put glucose where you’ll actually need it during the game, not buried in a bag across the room.Pick one win you’ll count as a success Monday morning, like “I treated every low with glucose first” or “I had fewer low‑then‑high spikes than last year.”Your goal isn’t a perfect graph, it’s less chaos than last Super Bowl. That absolutely counts.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. How To Stop Low Blood Sugars From Running Your Day
Show NotesIf January was about realizing how often lows hijack your day, February is about noticing how much that’s already changing. In this episode, we zoom out on the first month of the Low Blood Sugar Challenge and look at what’s actually different now: shorter lows, fewer roller‑coaster rebounds, and a little less panic every time the CGM screams at you.We talk about why progress with lows often shows up first in the shape of your days, not just the numbers on your graph, and how to spot the quiet wins you’re probably skipping right past. You’ll hear why “I still have lows” is not failure, how to tell if your new glucose‑first habit is working, and why this month is less about perfection and more about proving to yourself that you treat lows differently than you did in December. What You Can Do TodayLook back at one low from this week and one from before January and compare what actually happened, not just the numbers.Notice one way your response was even slightly less chaotic, even if the graph wasn’t perfect.Write one sentence that starts with “Now, when I’m low, I’m the kind of person who…” and finish it with what’s already true, not what you wish were true.Slow progress is still progress. Give yourself credit for the lows you’re handling better than before.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. What Panic Treating a Low Actually Costs You
Show NotesMost rebound highs don’t come from bad math or broken tech. They come from panic. In this episode, we talk about how quickly a low blood sugar can flip your brain into emergency mode, and why that urgency almost guarantees you’ll overshoot the fix. Not because you’re careless, but because your body is screaming for safety and speed, not precision. We walk through how panic treating turns a simple low into hours of cleanup, why it’s so hard to stop once you start eating, and what it looks like to treat lows with intention instead of fear. What You Can Do TodayThink about the last low where you ate faster than you planned. Notice how urgency, not hunger, drove the decision. Pick one low-treatment option you’ll use this week that slows you down just enough to stay measured.Calm doesn’t mean slow. It means deliberate.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. The Real Reason Lows Turn Into Overeating (Don't Be Mad At Me)
Show NotesMost people think overeating during a low is about hunger. It’s not. It’s about urgency. In this episode, we talk about why low blood sugar creates a sense of panic that feels exactly like hunger, even when your body doesn’t actually need more food. We break down how urgency overrides judgment, why “just a little more” keeps happening, and how one low can quietly derail your blood sugars for hours afterward. This isn’t about self-control. It’s about recognizing what’s really driving the behavior so you can interrupt it before it turns into another frustrating graph spiral. What You Can Do TodayNotice the difference between hunger and urgency the next time you’re low. Pay attention to speed, not appetite, how fast you want to eat. Decide on a single, measured low treatment you trust before urgency shows up.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. What Happens When You Wait Too Long to Treat a Low
Show NotesWaiting out a low feels responsible. Calm. In control. Until it isn’t.In this episode, we talk about what actually happens when you delay treating a low blood sugar, how quickly decision-making and coordination can slide, and why “I’ll just see if it comes back up” often ends in panic eating or a scary crash instead. This isn’t about being alarmist. It’s about understanding that lows have a window. Treating early isn’t weakness or overreacting, it’s how you stay ahead of the spiral that makes everything harder later. The goal isn’t to treat faster out of fear. It’s to treat earlier with intention. What You Can Do TodayThink about a time you waited to treat a low longer than you should have. Notice what you were hoping would happen if you just gave it a minute. Decide on a personal “treat by this number” rule so you’re not negotiating with yourself next time.Clear rules beat wishful thinking.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. Why Your Brain Fights Glucose Tablets (And How to Retrain It)
Show NotesIf glucose tablets work so well, why do so many people with type 1 diabetes avoid them like they’re punishment? In this episode, we dig into the psychology behind low blood sugar cravings. Why your brain wants comfort food instead of fast glucose, why tablets feel unsatisfying in the moment, and how past low experiences quietly train your reactions without you realizing it.This isn’t about forcing yourself to like glucose tabs. It’s about understanding why your brain resists them and how to slowly retrain that response so treating a low feels calmer, faster, and far less chaotic. When you change the association, the habit gets easier. What You Can Do TodayPay attention to what your brain asks for first during a low. Notice whether it’s about taste, comfort, or urgency, not just carbs. Practice pairing glucose tablets with reassurance instead of panic.Retraining starts with noticing.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. Why Your Brain Panics During a Low (And What to Do About It)
Show NotesLow blood sugar doesn’t just mess with your body, it hijacks your brain. That’s why perfectly reasonable people suddenly feel urgent, frantic, and out of control when their numbers drop.In this episode, we break down what’s actually happening in your brain during a low, why decision-making falls apart so fast, and why the instinct to overeat feels impossible to stop in the moment. This isn’t about discipline or willpower. It’s about biology doing exactly what it was designed to do… just not in a modern diabetes world.Once you understand why your brain reacts this way, you can build systems that work with it instead of expecting calm, logical choices when your glucose is crashing. What You Can Do TodayNotice how your thoughts change when your blood sugar starts dropping. Pay attention to urgency, fear, or “I need to fix this now” thinking. Decide one simple rule for lows ahead of time, so your brain doesn’t have to improvise.Plan beats panic.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. The Low Blood Sugar Rebound Nobody Warns You About
Show NotesIf you’ve ever gone from a low blood sugar straight into the 200s and thought, “How did that happen?”, this episode connects the dots.We talk about the post-low rebound that sneaks up on people with type 1 diabetes, not because they did something reckless, but because panic treatment feels necessary in the moment. This episode breaks down why lows often lead to highs, how stacking food during a low fuels the rebound, and why it feels so hard to stop once you start eating. This is about understanding the pattern, not blaming yourself for it. Once you see what’s happening, you can start treating lows in a way that keeps the rest of your day from getting hijacked. What You Can Do TodayLook at one recent low that ended in a big high. Notice how much you treated and how fast you ate it. Ask yourself, “What would a calmer version of that treatment have looked like?”Awareness comes first. Change comes second. Why Glucose Tablets Work Better Than Food for Low Blood Sugar
Show NotesMost people don’t hate glucose tablets because they don’t work. They hate them because they’re boring. No crunch. No comfort. No emotional payoff. In this episode, we talk about why glucose tablets actually outperform food when it comes to treating low blood sugar, and why using snacks instead quietly sets you up for rebound highs later. We break down how fast dextrose hits your bloodstream, why mixed carbs slow everything down, and how “real food” during a low turns into overeating almost every time. This isn’t about willpower. It’s about mechanics. When you understand what your body actually needs during a low, treating it stops feeling like a guessing game and starts feeling predictable again. What You Can Do TodayCheck what you usually grab first when you’re low, juice, candy, cookies, or snacks. Notice how often that turns into eating more than you planned. Decide on one low treatment this week that is fast, measured, and boring… on purpose.Boring works.ResourcesHelpful resources and newsletter: https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes. PBMs, Insulin, And The Broken System Keeping Prices High with George Huntley
Show NotesIf you have ever stared at your pharmacy receipt and thought, How is this even legal? this episode is for you. George Huntley has lived with type 1 diabetes since 1983, spent 25+ years running employer health plans, and now sits at the center of national advocacy work on insulin access and drug pricing. In this conversation, George breaks down how pharmacy benefit managers PBMs quietly took over drug pricing, why insulin became the poster child for a broken system, and how rebates turned into a giant, legal kickback loop that almost nobody at the patient level understands. We talk about why insurance started feeling so much worse in the 2010s, how PBMs profit when list prices go up, and why the health plan can actually pay less for insulin while you pay more at the counter.George also walks through what actually happens behind the scenes when your employer picks a health plan, why most employers have no idea how much money is flowing to PBMs, and why the drugs on your formulary are often chosen for maximum rebate, not lowest cost or best fit. We finish with a rapid-fire True or False round that exposes some of the most harmful myths about PBMs, insulin list prices, and whether transparency alone could fix this mess. What You Can Do TodayLearn the vocabulary PBM, rebate, formulary so you can ask sharper questions of your HR team, benefits broker, or insurer. If you work for an employer that offers health insurance, ask whether they know their PBM spread and rebate structure, and if they’ve ever audited it.Share this episode with one person who helps make benefits decisions at work, or with someone in your T1D circle who has been crushed by insulin costs and needs language for what they’re experiencing.ResourcesHelpful resources and newsletter:https://yourbestt1dyear.comSponsor ConnectBlue Circle Health offers free coaching and care that fits real life for adults with type 1 diabetes.
