Researchers have developed IMIREG, a novel 14-regulon transcriptional signature designed to predict how patients with various cancers will respond to immune checkpoint blockade (ICB). By utilizing a systems-biology approach to infer the activity of master transcription factors, this biomarker moves beyond simple immune cell counts to capture the actual regulatory circuitry of the tumor microenvironment. Validation across 50 clinical trial datasets demonstrated that IMIREG consistently identifies responders and outperforms established signatures like IFN-γ and T cell-inflamed profiles. Mechanistically, high IMIREG scores pinpoint an "immune-engaged but restrained" state, where active T cells and M1 macrophages are primed for reactivation by immunotherapy. The framework also reveals that highly coordinated dual-lineage immune programs often collapse during metastatic progression, explaining reduced treatment efficacy in advanced stages.
