This research article examines the role of the TIGIT immune checkpoint in driving resistance and relapse following BCMA-CAR-T therapy in patients with multiple myeloma. By utilizing single-cell RNA sequencing and flow cytometry, the authors demonstrate that high TIGIT expression on T cells correlates with increased tumor burden, poor prognosis, and accelerated T cell exhaustion. While in vitro inhibition showed limited results, TIGIT blockade using monoclonal antibodies in humanized mouse models significantly enhanced CAR-T proliferation and antitumor efficacy. Transcriptomic analysis further suggests that TIGIT modulates these effects by regulating cytokine pathways and the transcription factor BATF3.
