This research explores the immunological landscape of soft-tissue sarcomas (STSs), specifically focusing on the role of tertiary lymphoid structures (TLSs) in patient prognosis. By integrating immunohistochemistry with transcriptomic data, the study demonstrates that mature TLSs containing germinal center B cells are strong indicators of favorable clinical outcomes. The authors identify a novel 7-gene signature that links the presence of these structures to an inflamed tumor microenvironment and improved survival. A key finding is the opposing influence of two immune cell types: Th17-like cells promote TLS maturation, while immunosuppressive M2-like macrophages are associated with immature, less effective structures. Ultimately, combining signatures for B cells, Th17 cells, and low macrophage infiltration creates a highly accurate tool for predicting patient recovery.
