This research identifies CD147-positive migrasomes as a critical driver of sorafenib resistance in hepatocellular carcinoma. The study demonstrates that hypoxic conditions trigger the release of these large extracellular vesicles, which are internalized by cancer cells through LPAR6-dependent macropinocytosis. Once absorbed, these migrasomes activate the PI3K/AKT/TWIST1 signaling pathway, promoting vasculogenic mimicry to provide alternative blood supply for tumor survival. The authors conclude that dual-targeting of the uptake process and mimicry formation could effectively restore the efficacy of sorafenib.
