If you've been part of a physician family for any length of time, you know what it's like to believe life will finally begin after the next milestone. First it's medical school, then residency, fellowship, or that long-awaited attending job. We tell ourselves that once we get through this season, life will slow down, we'll have more time together, and many of the sacrifices we've made along the way will finally feel worth it. The reality is often more complicated. When my husband finished residency in 1999, I thought we had reached the finish line. Instead, we found ourselves standing at the beginning of an entirely new chapter—one filled with opportunities, certainly, but also with decisions we had never been taught how to make. After years of living on a resident's salary, we suddenly had to navigate student loans, retirement planning, taxes, investing, and the quiet pressure to build a life that looked like we thought a physician's life was supposed to look. Looking back nearly thirty years later, I can honestly say we made many of the same mistakes we discuss in this conversation. That's why I wanted to invite James Nutter to the podcast. James is a Partner and Financial Planner at IM Wealth, and while he spends his career helping physician families build financial confidence, our conversation quickly became about something much deeper than money. We explored what it means to live intentionally after years of delayed gratification, why so many physician families continue to feel financially overwhelmed despite earning excellent incomes, and how our financial decisions often reflect something much bigger than dollars and cents—they reflect our priorities, our values, and the kind of life we're creating together. One idea James shared has stayed with me ever since we recorded this episode. He described how easy it is to build an "accidental financial life," making one reasonable decision after another without ever stepping back to ask where those decisions are leading. The more I reflected on that idea, the more I realized it applies to medicine just as much as it applies to money. Physician families become incredibly skilled at adapting to whatever comes next, but adaptation isn't the same thing as intention. At some point, we have to pause long enough to ask whether the life we're building actually reflects the one we've spent so many years working toward. We also talk about one of the most overlooked aspects of financial wellness: communication. James shares practical advice for making money conversations a regular part of your relationship instead of something reserved for moments of stress or conflict. I loved this part of our conversation because healthy communication isn't just good for your finances—it's one of the strongest protective factors for a healthy medical marriage.
