42. The Power of the Pause: Mindfulness for Physicians in Micro-Moments | Rashmi Bismark, MD, MPH

from The MedLife Support Podcast · host Lisa Muehlenbein, PhD

A physician may leave the hospital, clinic, or call shift—but that doesn't mean their nervous system has left work behind. Medicine asks physicians to function through fatigue, hunger, discomfort, urgency, emotional intensity, and constant stimulation. Over time, pushing through can become so automatic that it becomes difficult to recognize what the body is signaling at all. And then the physician walks through the front door. In this episode of The MedLife Support Podcast, Dr. Lisa sits down with Rashmi Bismark, MD, MPH, a preventive medicine physician, mindfulness and meditation teacher, Yoga Medicine teacher trainer, and children's book author, to explore what can happen when chronic stress becomes the body's normal—and how small moments of awareness can begin to interrupt the pattern. Rashmi challenges the assumption that meditation requires sitting quietly for 30 minutes or adding another item to an already impossible schedule. Instead, she introduces micro-moments of meditation: brief, intentional pauses woven into activities that are already happening throughout the day. That might mean feeling your feet before walking into a patient's room. Noticing one breath. Paying attention to the sounds around you. Pausing between the car and the front door. Or taking a few seconds to recognize a moment of care rather than rushing past it. The goal isn't perfection. And mindfulness isn't a Band-Aid for the systemic conditions driving physician burnout. It is an opportunity to notice. Because when physicians become more aware of what is happening inside themselves, they may also become more aware of how that stress is showing up with the people they love. Lisa and Rashmi explore the transition from work to home, why rituals can help physicians shift between those two worlds, and how mindfulness becomes relational—not just individual. They also discuss what happens when spouses absorb stress secondhand, why partners need permission to ask for connection, and how simple family practices can give everyone more room to voice what they need. In this episode, you'll hear: What chronic stress and hypervigilance can look like in physicians How stress can show up as agitation, fatigue, withdrawal, or irritability at home Why medicine can teach physicians to override important physical and emotional cues What micro-moments of meditation actually are Simple mindfulness practices physicians can use during a clinical day How to create a work-to-home transition ritual Why mindfulness should never be positioned as the fix for systemic burnout How sensory awareness can provide an accessible starting point when focusing on the breath feels difficult Why moments of care and compassion are worth noticing and savoring How mindfulness practices can involve spouses, children, and the whole family Why asking your partner for a moment of connection matters The value of family meetings in high-demand medical households Why community and belonging matter when physicians are struggling Rashmi's advice for staying connected through the chaos of medical life Sometimes the shift doesn't begin with an hour-long practice. Sometimes it begins with one pause between where you've been and who you want to be when you walk through the door.   About Dr. Rashmi Bismark Rashmi Bismark, MD, MPH is a mindfulness meditation teacher, yoga educator, and board-certified preventive medicine physician whose work bridges modern medicine, contemplative practice, and lived human experience.
