Lichen planus is an inflammatory condition that causes red lesions on or near the head of the penis. These lesions come and go in the same spot on the penis and are often flaky with sharp or jagged edges. Lichen sclerosis is a similar but more severe condition that can cause serious complications, including buried penis. Lichen sclerosis is treated by removing the affected skin and replacing it with grafts of healthy tissue to prevent the otherwise inevitable recurrence of disease. Early treatment of lichen planus can help prevent the development of lichen sclerosis. This listener explains that the red patch on his penis goes away when he uses a Protopic ointment, but only for 4 or 5 months at a time. During flare ups, he avoids sex because the friction is painful. He also worries about the side effects of this prolonged medication use and asks if there are other treatment options he should consider. Dr. Mistry explains that creams or ointments like betamethasone are helpful in treating lichen planus lesions, especially when mixed with lidocaine to reduce pain. Another option for those with lichen planus is the injection of PRP, or platelet rich plasma, into the affected skin. While this cutting edge treatment isn't covered by insurance, it costs only $750 for a one-time, in-office injection.
