This report, intended to review the welfare system's compliance framework, was found to contain numerous errors, including fabricated quotes, nonexistent references, and false citations, which were attributed to the use of generative artificial intelligence. Consequently, Deloitte agreed to issue a partial refund to the government for the substandard work. The news coverage emphasizes that this incident highlights the limitations of AI systems, particularly the tendency for "hallucinations" where the technology invents information. A senator criticized Deloitte for having a "human intelligence problem."
