Peak incidence: 6–18 months, mostly under 4 years.🔬 Causes• Bacterial: Streptococcus pneumoniae (↓ with vaccine), Haemophilus influenzae (not Hib), Moraxella catarrhalis.• Viral triggers: RSV, rhinovirus, influenza, adenovirus—often set the stage for bacteria.🚸 Risk Factors• Young age, nursery attendance, passive smoking, recent URTI, allergies, chronic sinusitis, cleft palate, bottle feeding flat, family history.⚙️ Pathophysiology• Viral URTI disrupts the nasopharynx → bacteria ascend eustachian tube → inflammation and fluid/pus in middle ear → pain & possible perforation.• Young children: eustachian tube is shorter/flatter—easier bug entry!🩺 Clinical Features• Symptoms: Sudden ear pain (otalgia), fever (in ~50%), irritability, hearing loss. Babies may pull at ears, be unsettled or feed poorly.• Signs: Bulging, red, or cloudy tympanic membrane (ear drum), reduced mobility, possible discharge if perforated. In teens/adults: may just present with ear pain.🔍 Diagnosis & Differentials• Diagnosis is clinical: based on history and otoscopy.• Differentials: OME (glue ear), otitis externa, mastoiditis, TMJ dysfunction, dental pain, trigeminal neuralgia, migraine, referred pain (more common in adults).💊 Management• First-line: Pain relief (paracetamol/ibuprofen). Warm compress may help.• Antibiotics: Only if severe, persistent >4 days, systemically unwell, <2 years bilateral AOM, perforation/discharge, or high risk—amoxicillin first choice.• Watchful waiting is safe for many, especially mild cases. Safety netting advice is vital!• Admit if: <3 months with fever ≥38°C, <6 months with fever ≥39°C, severe/systemic illness, complications or high-risk patients.🩹 Complications• Tympanic membrane perforation, mastoiditis, CSOM (chronic suppurative OM), OME (glue ear), hearing loss, facial nerve palsy, labyrinthitis, meningitis, brain abscess, rarely systemic sepsis or endocarditis.• Follow-up for persistent discharge >6 weeks, or recurrent AOM (≥3 in 6 months or ≥4 in 12 months)—ENT referral.🦻 Prognosis• Most resolve within 3–7 days.
