In occasione del Congresso Europeo per il Trattamento e la Ricerca sulla Sclerosi Multipla, la GJCF, un’associazione di pazienti americana volta allo studio e alla ricerca delle patologie demielinizzanti anticorpo-mediate, ha organizzato un pomeriggio di update sulla terapia e gli unmet needs di NMOSD (Neuromyelitis optica spectrum disorder) e MOGAD (Myelin oligodendrocyte glycoprotein antibody-associated disease). Sono stati discussi i recenti trials terapeutici nei nuovi farmaci volti al trattamento dell’NMOSD: l’eculizumab che ha come target il complemento, l’inebilizumab che è un anticorpo monoclonale anti-CD19 e il satralizumab, un anticorpo monoclonale che ha come target il recettore dell’interleuchina 6. È stato poi discusso il prossimo aggiornamento dei criteri diagnostici dell’NMOSD, resosi necessario soprattutto per il miglior inquadramento delle forme sieronegative. Sono stati inoltre discussi i quattro unmet needs ritenuti di maggior rilievo: 1. L’accesso globale ai test sierologici ed alle terapie; 2. L’attività di malattia indipendente dalle ricadute; 3. La consapevolezza e l’educazione di medici e pazienti; 4. Il riconoscimento e la gestione delle disparità. Infine, sono stati presentati alcuni casi clinici che hanno stimolato la discussione sulle scelte terapeutiche e gestionali dei pazienti con NMOSD e MOGAD.
