Are you responsible for how things went? The way we typically operate or relate to responsibility is that it is a fact. Whether you are responsible or not, there are problems with this approach. Some of the problems are that when responsibility is a fact, then taking responsibility for how things went becomes a fact-finding mission. As a leader, it diminishes your capacity to be responsible. It also lowers your ability to affect change.
