Ongoing follow-up and re-engagement with cardiac patients should occur at minimum every 12 months, with more frequent support needed for isolated patients or those struggling with behavior change (such as smoking cessation). A dedicated cardiologist with specialized interest in secondary prevention should lead rehabilitation programs to ensure consistent, evidence-based guidance toward specific health targets for each patient.
