Left main coronary artery disease requires treatment regardless of plaque quality, as it represents the weakest link in coronary circulation and carries significant risk. Cholesterol-lowering therapy (statins) stabilizes fatty plaque and converts it to calcified plaque, making it difficult to assess plaque progression on repeat calcium scoring but more visible on contrast-enhanced CT angiograms.
