EP293: Heart Rate Variability

from Doctor Warrick Bishop - Heart Health · host Doctor Warrick

Welcome to my podcast. I am Doctor Warrick Bishop, and I want to help you to live as well as possible for as long as possible. I’m a practising cardiologist, best-selling author, keynote speaker, and the creator of The Healthy Heart Network. I have over 20 years as a specialist cardiologist and a private practice of over 10,000 patients. Podcast Summary Introduction Dr. Warrick Bishop, a cardiologist, author, keynote speaker, and CEO of the Healthy Heart Network, hosts this episode focused on helping people understand cardiovascular health. In this episode, Dr. Bishop explores the emerging topic of heart rate variability (HRV), explaining that while he doesn't routinely use it clinically, it's increasingly appearing in wearable devices and social media, prompting him to research and demystify this complex measurement for his audience. Key Takeaways Heart rate variability measures the subtle, constant changes in the intervals between consecutive heartbeats, reflecting the body's biological complexity and the interplay between the nervous system and the heart. The autonomic nervous system plays a crucial role in HRV through two opposing systems: the sympathetic (fight-or-flight) system that accelerates the heart, and the parasympathetic (rest-and-digest) system that slows it down. Heart rate variability indicates the heart's ability to adapt to changing environmental conditions—good HRV means the heart can quickly accelerate or decelerate as needed by the body. HRV measurement is far more complicated than simple soundbites suggest, involving complex mathematical chaos models and multiple assessment methods that must be measured consistently (24-hour, 5-minute, or shorter periods). The same HRV value can indicate different health outcomes depending on context; for example, increased HRV in atrial fibrillation patients is a bad sign linked to increased mortality, while decreased HRV may indicate autonomic nerve problems. Medications significantly impact HRV measurements, making interpretation challenging for individuals taking drugs that affect the autonomic nervous system. Personal wearable devices and smartphones increasingly measure HRV, but standardized baselines and consistent measurement protocols are needed to meaningfully compare data across different devices and time periods. Future applications of HRV monitoring could include early detection of developing infections, like identifying the onset of a cold before symptoms appear.
