Why do we seek to blame someone or say it is Human Error without  firstly using a systems thinking approach to analyse the influencing  factors surrounding an incident?Sharon Todd discusses 'Blame' and what we can 'Learn' from a Systems Thinking  approach with Gemma Read.Associate Professor Gemma Read is the Director of the Centre for Human Factors and Sociotechnical Systems and an Associate Professor in Psychology at the University of the Sunshine Coast. She has degrees in behavioural science and law and completed her PhD at Monash University. She has over 16 years’ experience applying human factors and systems thinking methods in both academia and government roles.
