Integrating the 4Ms Framework into the EHR via the FlourishCare Model – University of Louisville Trager Institute GWEP

The University of Louisville Trager Institute GWEP case study discusses integration of the 4Ms Framework into the EHR system through their innovative FlourishCare (FC) model.  This initiative transformed their geriatrics primary care clinic into an interprofessional, age-friendly hub by embedding a holistic, biopsychosocial assessment directly into the EHR, enabling the creation of shared, goal-oriented care plans centered on what matters most.
