Living Longer Isn’t Enough: The Black Man’s Guide to Healthspan After 40 with Dr. Noelle Reid

I have been thinking a lot about what it means to grow older as a Black man. My father lived to 93, but vascular dementia changed the final years of his life. My mother is now 85 and living with Alzheimer’s. Those experiences have forced me to ask a harder question. I do not simply want to live a long time. I want to live well. That is the difference between lifespan and healthspan. In this conversation with board-certified family physician Dr. Noelle Reid, we unpack what Black men over 40 need to start paying attention to now if we want to remain strong, sharp, sexually healthy, mobile, and independent well into our 60s, 70s, and 80s. Dr. Reid makes one thing clear: what your health looks like at 60 is being built at 40. Waiting until something hurts, your energy disappears or your body stops performing the way it once did puts you behind the curve. We talk about becoming more medically literate and asking for health markers that may go beyond the standard physical, including ApoB, lipoprotein(a), fasting insulin, inflammatory markers, and a more complete look at thyroid and hormone health. We also go directly into a conversation many men avoid: andropause. Lower testosterone, changing estrogen levels, loss of muscle, fatigue, changes in mood, decreased libido and erectile changes can all be signals that something is shifting. The answer is not automatically testosterone replacement therapy. The first step is understanding your numbers, your symptoms and what is actually happening in your body. Dr. Reid also challenges the way many of us think about fitness. Longevity is not just how much weight you can lift or how hard you can go in the gym. Core strength, lower-body strength, spinal mobility, balance, and flexibility all matter. That is why we spend time talking about yoga, Pilates, and something many high-performing men struggle with: stillness. One of my biggest takeaways from this conversation was simple: The goal is to balance movement with stillness. Exercise. Build strength. Eat enough protein. Get your vitamin D checked. Protect your sleep. Pay attention to your hormones. Learn how to breathe. Learn how to recover. Most importantly, stop waiting for your body to break before you start taking care of it. For Black men over 40, longevity cannot simply mean adding more birthdays. I want us hiking, traveling, making love, building businesses, playing with our grandchildren, and creating impact for decades to come. That requires us to stop asking, “How long will I live?” The better question is: How well will I live while I am here? That is the revolution.
