LT4 vs Combination Therapy vs DTE

This episode compares the effectiveness of levothyroxine (LT4), combination therapy (LT4+LT3), and desiccated thyroid extract (DTE) in treating adults with primary hypothyroidism. The 2021 crossover trial by Shakir & Hoang et al. evaluated 75 hypothyroid patients across three 22-week treatment arms: Levothyroxine (LT4) monotherapy, LT4 + Liothyronine (LT3), and Desiccated Thyroid Extract (DTE). However, a subgroup of the most symptomatic patients at baseline reported a strong preference and significant symptom reduction on T3-containing regimens.ConclusionFor the aggregate population, DTE and combination therapies offer no clinical advantage over standard LT4 monotherapy. T3 options remain a targeted alternative for individuals with persistent residual symptoms.
