Major failures by hospital staff and police resulted in the girl being handcuffed, admitted to a Waikato Hospital ward, restrained, and twice injected with anti-psychotic medications. The girl's family say they can't understand how she was mistaken for a 20-year-old woman. Samantha Gee reports.
