PictureIn 2014, Formé opened a brand new 11,000 square foot medical facility in White Plains, New York, doing business as Formé Urgent Care and Wellness Center. This state-of-the-art facility operates as a full service Diagnostic and Treatment Center that provides the following services; Walk-In Medical Care, Physical and Occupational Therapy, Radiology, Diagnostics and Outpatient Surgery. Formé is opened 365 days a year and is overseen by board certified emergency medicine physicians.
