We talk a lot about "Modern Medicine"—the robots, the lasers, the miracle drugs. But today, I want to talk about the "Low-Tech" hole in the system that is actually putting lives at risk. It’s the gap between the specialist and your PCP doctor. We call it "Care Fragmentation," but you probably know it as: "The right hand doesn't know what the left hand is doing.”Research into the American healthcare system highlights a few startling statistics regarding these gaps:Communication Failures: Studies have shown that roughly 25% to 50% of PCPs are unaware of what happens during their patients' specialist visits.Referral Loop Gaps: In many systems, the "referral loop" (the specialist sending a report back to the PCP) is only completed about half the time.Medication Errors at Discharge: Nearly 20% of patients experience an adverse event within three weeks of leaving the hospital, and the vast majority of these are related to medication changes that weren't properly reconciled or monitored by the follow-up with their PCP.
