In this episode, weexplore the Ottawa Ankle Rules — a clear,evidence-based clinical tool that helps healthcare professionals decide when imaging is necessary after ankle injuries. 🦶What Are the OttawaAnkle Rules?A clinical decision rule designed to helpdetermine if a patient with ankle or foot painneeds an X-ray, based on a focusedexamination.They apply topatients ≥2 years old with acute ankle or midfoot injuries. 🔍Key Criteria for anAnkle X-rayOnly consider theserules if there is pain in the malleolar zone(around the ankle bones).Then, X-ray isrecommended if any of the following arepresent:Bone tenderness at the posterior edge or tip of the lateral malleolus (outer ankle) — within 6 cmBone tenderness at the posterior edge or tip of the medial malleolus (inner ankle) — also within 6 cmInability to bear weight for four steps immediately after injury and during clinical assessmentEven limping counts as weight-bearing 🦶Key Criteria for aFoot X-rayOnly consider ifthere is midfoot pain, plus:Tenderness at the base of the 5th metatarsal (lateral edge of foot)Tenderness at the navicular bone (medial arch)Inability to bear weight for four steps 🧠Mnemonic to Remember“Laterally or medially touchy OR can’t take foursteps”Covers both outer (lateral) and inner (medial) bone tendernessAnd the functional weight-bearing test ⚖️Why Are These RulesImportant?Reduce unnecessary X-rays and radiationImprove emergency department efficiencyBased on high-quality evidenceSupport smarter resource use in healthcare ⚠️When Not to Use theRulesClinical judgmentalways comes first.
