🎧 FREE MSRA PODCAST – Paget’s Disease of Bone: Diagnosis, Features & ManagementIn this MSRA-focused deep dive, we unravel Paget’s disease of bone — a commonly tested but often overlooked metabolic bone disorder. Learn to spot classic clinical clues (like bone pain and deformity), understand how it differs from osteoporosis, and revise NICE-aligned management strategies in under 5 minutes.🧠 Key Learning Points📌 Definition• Paget’s disease = a chronic bone remodelling disorder.• Characterised by focal regions of excessive bone turnover → disorganised bone structure.• Commonly affects the pelvis, spine, skull, femur, and tibia.• Can be monostotic (one site) or polyostotic (multiple).📌 Causes & Risk Factors• Idiopathic (exact cause unknown).• Genetic predisposition: mutations in SQSTM1 gene.• Environmental: possible viral triggers (e.g. paramyxovirus).• Risk ↑ with age, more common in men.• Often asymptomatic and picked up on imaging or bloods.📌 Pathophysiology• 3 stages: 1. Lytic phase – osteoclast overactivity → bone resorption. 2. Mixed phase – osteoblasts increase → disorganised repair. 3. Sclerotic phase – dense but weak bone formation.• Results in expanded, structurally weak, and deformable bones.📌 Clinical Features• Often asymptomatic.• Bone pain – most common symptom.• Bone deformity – e.g. bowed legs.• Fractures – due to structurally weak bones.• Enlarged skull → increased hat size or hearing loss from cranial nerve compression.• Spinal involvement → back pain, radiculopathy.• May present with high ALP on routine bloods.📌 Differential Diagnoses• Osteoporosis (but without bone expansion).• Bone metastases.• Myeloma.• Osteomalacia.• Hyperparathyroidism.📌 Investigations🩸 Bloods:• Isolated raised ALP (often >200 IU/L).• Normal calcium and phosphate.🩻 X-ray:• Coarsened trabeculae, bone enlargement, cortical thickening.📸 Bone scan:• Shows extent and activity (hot spots).💡 MRI or CT if complications (e.g. malignancy) suspected.📌 Management (UK NICE-aligned)• Asymptomatic → monitor (no treatment if ALP stable).• Symptomatic or high-risk: – First-line: IV zoledronic acid (most effective). – Alternative: oral risedronate if IV not suitable.• Pain management: NSAIDs/paracetamol.• Surgical: for fractures, deformities, spinal stenosis.• Monitor ALP every 6–12 months post-treatment.📌 Complications• Fractures.• Osteoarthritis in affected joints.• Nerve compression (e.g. deafness, radiculopathy).• Rarely: osteosarcoma (malignant transformation).📌 Prognosis✅ Good with early diagnosis and bisphosphonate treatment.⚠️ Complications more likely if left untreated or widespread disease.📚 MSRA Resources – Paget’s Disease of Bone📝 Revision Noteshttps://www.passthemsra.com/topic/pagets-disease-of-bone-revision-notes/🃏 Flashcardshttps://www.passthemsra.com/topic/pagets-disease-of-bone-flashcards/📖 Accordion Q&A Noteshttps://www.passthemsra.com/topic/pagets-disease-of-bone-accordion-qa-notes/🎯 Rapid Quizhttps://www.passthemsra.com/topic/pagets-disease-of-bone-rapid-quiz/🧪 Quizhttps://www.passthemsra.com/quizzes/pagets-disease-of-bone/🌐 More MSRA Supporthttps://www.passthemsra.comhttps://www.freemsra.com💬 Final ThoughtDon’t confuse Paget’s with osteoporosis — think raised ALP, bone expansion, and hearing loss in older adults. It’s an MSRA classic.
