NETest

The NETest is a multigene liquid biopsy measuring 51 circulating transcript profiles via RT-PCR and machine-learning algorithms to quantify neuroendocrine tumor activity on a 0 to 100 scale. Capturing programs like proliferation, somatostatin receptor expression, and epigenomic signaling, it yields superior diagnostic sensitivity (89-99%) and progression accuracy compared to chromogranin A. Clinically, it anticipates RECIST-defined recurrence months prior to imaging and stratifies post-surgical residual disease. However, variable specificity in non-NET malignancies, lack of NCCN endorsement, single-lab proprietary architecture, and absent prospective outcome-driven data currently limit its utility to adjunct surveillance rather than diagnostic screening or direct treatment selection.
