Resistance to endocrine therapy in patients with hormone-receptor-positive (HR+), HER2-negative breast cancer inevitably results in relapse and metastasis, leading to death. Understanding mechanisms of intrinsic and acquired resistance have been the focus of many clinical studies. Recent advances including the use of cyclin-dependent kinase 4/6 (CDK4/6) inhibitors and phosphoinositide 3-kinase (PI3K) inhibitors have improved progression-free survival and overall survival in these patients, however, endocrine resistance still develops in some patients. New classes of agents including oral selective estrogen receptor degraders (SERDs) and CDK7 inhibition have been explored in advanced HR+/HER2- metastatic breast cancer.
