In part 1 of this series, the topics of healthcare delivery transformation and “near misses” are discussed. With new technologies, such as telehealth, drones, robotic surgery, 3d printing, and more, there are new possibilities for delivering healthcare. Dr. Michael G Knight explains his role as head of healthcare delivery transformation. He explains that we don’t live in a society where everyone has the same opportunity to care, so to transform healthcare we have to think about innovative ways of delivering health to all patients. An important part of delivering care is patient safety. Patient-based science ensures that there are checkpoints to prevent error. Dr. Knight uses the term swiss cheese model to explain how policies are layered to create barriers that prevent errors from reaching patients. In patient safety models, there are several domains. These domains include the patient, healthcare professional, and therapeutic system. These domains work together; therefore, methods and policies are in place among all to ensure patient safety. As a patient safety practitioner, the goal is to prevent the adverse effect from ever reaching the patient. This is a proactive risk assessment. Dr. Sogade asks what happens when something does go wrong. Dr. Knight uses “just culture” - looking at the underlying issue and holding people accountable when necessary. He discusses the importance of solving the underlying issue if applicable in order to prevent recurring problems.
