This episode breaks down HerpesZoster Ophthalmicus, the eye-threatening reactivation of VZV, into clear, fast, and clinically sharpinsights. 🧠Key Learning Goals:👁️ What is HZO?➡️ It’s the reactivation of varicella-zoster virus (the chickenpox virus) in the ophthalmic (V1) branch of the trigeminal nerve.➡️ This leads to painful rashes,ocular inflammation, and in some cases, serious vision loss. ⚡Who gets it?📌 Must have had chickenpox before.📌 Risk ↑ with:Ageing (immunosenescence)Immunosuppression (HIV, chemotherapy, steroids, etc.)Stress or chronic illnesses💡Mnemonic:“Age and Immune Issues Make Chickenpox HZO” 🔬Pathophysiology Recap:🦠 Dormant VZV in thetrigeminal ganglion reactivates → travels along V1 → causes inflammation + vesicular rash in the forehead, eyelid, and eye.🔥 The nasociliary branchinvolvement = higher risk of serious eye damage. 🧾Symptoms:🩹 Prodromal pain before rash👁️ Eye redness, watering, photophobia🔥 Vesicular rash on one side of the face (respects the midline)👃 Hutchinson's sign = rash on the nose tip = ⚠️ likely eye involvement 🩺Differentials:❌ Bacterial/viral conjunctivitis❌ Corneal ulcers❌ Sinusitis✅ But dermatomal vesicular rash in V1?
