Get clear on causes, symptoms, management, and why prevention is so crucial.🧠 Key Learning Points📌 Definition & Types• VKDB (Vitamin K Deficiency Bleeding) = Bleeding disorder in newborns due to low vitamin K• Also known as haemorrhagic disease of the newborn• Subtypes by onset: – Early: <24h (linked to maternal meds) – Classic: Day 1–7 (most common) – Late: Weeks 2–12 (highest risk of brain bleed)📌 Why Newborns Are At Risk• Born with naturally low vitamin K (low placental transfer, sterile gut, little dietary intake)• Breast milk = very low vitamin K• Gut bacteria (which make vitamin K) not yet established• Certain maternal medications (anticonvulsants, warfarin, antibiotics) increase risk📌 Pathophysiology• Vitamin K is essential for activating clotting factors II, VII, IX, X, plus proteins C & S• Deficiency = underactive clotting system → risk of serious bleeding📌 Risk Factors• Exclusive breastfeeding (without vitamin K supplementation)• Maternal drug use (see above)• Prematurity (immature liver, delayed feeding)• Underlying liver/gut disease, malabsorption📌 Clinical Presentation• Early: Bleeding within 24h (often severe, linked to maternal meds)• Classic: Bleeding days 1–7 (GI bleeding, umbilical stump, skin, after procedures like circumcision)• Late: Weeks 2–12, often with sudden intracranial haemorrhage (seizures, lethargy, pallor, bulging fontanelle), GI or skin bleeding• Red flags: Bleeding from multiple sites, unexplained bruising, CNS signs📌 Differential Diagnosis• DIC, sepsis, thrombocytopenia, trauma (including non-accidental injury), inherited coagulopathies (e.g. haemophilia), liver disease📌 Diagnosis• History: Breastfeeding, vitamin K prophylaxis at birth, maternal drugs, prematurity• Lab findings: Prolonged PT/INR (key!), often prolonged APTT, normal platelets• Response to vitamin K confirms diagnosis📌 Management & Prevention• Give vitamin K!
