Population Health Management bitesize #2: Setting up effective structures in Leeds

In this episode we're joined by Jenny Cooke from Leeds Health and Care Partnership along with Claire Turner from Carers Leeds. They were on wave one of the NHS England Population Health Management Development Programme and were also part of the Place Development Programme this year. They have been working closely to embed population health management within their system and talk to us about how this structure works and how relationships with the third sector have been developed.
