What hooked us in this conversation is the “how” a millisecond pulse becomes a durable shift in depression, anxiety, and even the stuckness of rumination and anhedonia, and why that maps so cleanly onto positive psychiatry and the pursuit of wellness, not just fewer symptoms.We start with the core biophysics: a rapidly changing magnetic field crosses the skull and, by Faraday’s law, induces an electric field in the superficial cortex. From there, we build a practical framework for understanding transcranial magnetic stimulation across nested levels of biology and time scales, from immediate cortical depolarization to circuit engagement in the dorsolateral prefrontal cortex, to large-scale network reorganization in systems like the default mode network and cognitive control network. Along the way, we make a key distinction that clears up a lot of confusion: the stimulation site is often an entry point, while the therapeutic target is the network.Then we get specific about what makes protocols different and why “dose” is more than minutes under the coil. We talk frequency as a biological message, including classic 10 Hz rTMS, inhibitory low-frequency approaches, and theta burst stimulation that can compress treatment while staying biologically efficient. We also zoom down to synapses and molecules, connecting glutamate signaling, NMDA and AMPA dynamics, calcium cascades, and BDNF-driven neuroplasticity, plus the roles of dopamine and GABA in motivation, reward, and the excitation-inhibition balance.
