McKinsey delivered a warning report in April 2013. Senate investigators found that dozens of HHS officials and hundreds of contractors knew about critical gaps in testing months before launch. Red flags were raised — and ignored. Political pressure from the White House ensured the site went live on schedule, regardless of whether it was ready.
