Relapse

from Recovery Decoded · host Recovery Decoded

Relapse does not start when you pick up the substance. It starts days or weeks before. And if you know what the early dominoes look like, you can catch them before the last one falls. This is the capstone episode of the season. Every tool from twelve episodes combined into one system. We cover:• The domino sequence — the specific behavioral, emotional, and cognitive warning signs in the order they typically appear before a relapse• Why "I've got this" is the most dangerous thought in long-term recovery• The prefrontal cortex fatigue cycle — when you stop doing the protective behaviors, your ability to maintain them decreases further• Marlatt's relapse model and why the cascade starts BEFORE the high-risk situationTHE FOUR STAGES OF RELAPSE:Stage 1 — Behavioral erosion (days to weeks before): skipping routine, isolating, diet collapsing, environment slidingStage 2 — Emotional warning signs (days before): irritability, "I don't care," romanticizing, shame spiral returningStage 3 — Cognitive distortions (hours to days before): "one time won't hurt," "I've earned this," "nobody will know"Stage 4 — Behavioral setup (hours before): driving the old route, carrying cash, being alone, going somewhere "for a different reason"LAPSE vs RELAPSE:A lapse is a single use event. A relapse is a return to the pattern. The difference matters. The abstinence violation effect — "I already ruined it" — is what turns a stumble into a collapse. It is shame hijacking your decision-making, not logic.THE FULL RELAPSE AUTOPSY (6 steps):1. Stabilize first (wait 3-7 days, not 24 hours)2. Map the dominoes backward with a counselor/sponsor3. Identify the intervention point (almost always in Stage 1)4. Build the if-then plan: specific, written, pre-decided5. Share the plan: "I'm giving you permission now because I won't in the moment"6. File it — patterns repeat, and documented patterns are interruptible patternsTHE EMERGENCY PROTOCOL (all season tools in order):1. HALT (EP3)2. 5-4-3-2-1 grounding (EP4)3. Box breathing (EP3)4. 15-minute delay (EP4)5. Urge surf (EP4)6. Change physical state (EP4)7. Cold stimulus (EP3)8. Call someone (EP12)THE WEEKLY RADAR CHECK:Run through Stage 1 and Stage 2 signs weekly. If 2+ are present — intervene today, not tomorrow.REFERENCES:• Marlatt GA, Gordon JR (1985).
