S10 E3 - Building Clinical Confidence Through Reflective Practice

from Stop F*cking Settling! Hosted by Sarah Collison, Nikki Cousins and Alyce Svensk

Why great supervision starts with understanding what new OTs don't know Supporting another Occupational Therapist to develop their clinical skills is rewarding, but it isn't always straightforward. Many experienced clinicians have spent years refining their practice, making countless clinical decisions so often that the reasoning behind them has become automatic. This creates a challenge during supervision. The very skills that make someone an experienced clinician can make them harder to teach. We forget what it was like to learn Experience changes the way we think. Tasks that once required conscious effort eventually become second nature. We instinctively identify risks, adapt our communication and prioritise interventions without stopping to think through every decision. Learning to drive offers a useful comparison. An experienced driver doesn't consciously think about checking mirrors, indicating or positioning the car. A learner, however, needs every step explained. Clinical practice develops in much the same way. When supervisors assume these skills are obvious, new clinicians can miss the thinking that underpins good practice. Reflective competence makes the invisible visible This is where reflective competence becomes so valuable. Reflective competence is the ability to pause and examine the reasoning behind clinical decisions. Instead of relying on instinct, clinicians consciously explore why they made a particular choice, what information influenced that decision and how they might approach a similar situation in future. Making this reasoning explicit helps transform knowledge that has become automatic into learning that can be shared. Better supervision starts with better conversations Observation is an important part of supervision, but simply watching an experienced OT isn't enough. New clinicians can usually see what is happening. What they often miss is why decisions are being made. Explaining why you changed your questioning, noticed a safety concern or selected one intervention over another helps develop clinical reasoning rather than encouraging clinicians to simply copy behaviour. This approach also benefits experienced practitioners by encouraging ongoing reflection and continual professional growth. Confidence grows through supported practice Confidence doesn't come from being left to work independently before you're ready. It develops through observation, discussion, guided practice and constructive feedback. Breaking complex clinical tasks into manageable steps helps clinicians build competence progressively. As confidence grows, supervision naturally shifts from direct instruction towards collaborative reflection and independent clinical judgement. Creating a workplace where questions are encouraged and learning is expected benefits clinicians at every stage of their career. Reflective practice strengthens the whole profession Reflective competence isn't only for new graduates. Every OT benefits from regularly examining their own practice and remaining curious about how they make clinical decisions. When experienced clinicians make their thinking visible, supervision becomes more meaningful, onboarding becomes more effective and teams develop stronger clinical capability. Ultimately, great supervision isn't about having all the answers – it's about helping others understand how good clinical decisions are made. Key takeaways for OTs • Effective supervision requires more than demonstrating clinical skills. • Unconscious competence can make important reasoning invisible to new clinicians. • Reflective competence helps experienced OTs explain how they make clinical decisions. • Observation is most valuable when paired with discussion and reflection. • Confidence develops through supported practice, feedback and gradual independence. • Strong supervision builds clinical reasoning, not just clinical skills.
