Prof. Alessandro Tessitore e Prof.ssa Federica ProviniI disturbi del sonno sono uno dei sintomi non motori più comuni della Malattia di Parkinson (MP), ad impatto negativo sulla qualità di vita dei pazienti, dei loro caregivers e sull’efficacia della terapia. L’insonnia è molto frequente, interessando fino all’81% dei pazienti. E’soprattutto un’insonnia di mantenimento, caratterizzata da difficoltà a mantenere la continuità del sonno e risvegli frequenti. Fluttuazioni motorie, tremore, rigidità e difficoltà a girarsi nel letto sono spesso le cause dell’insonnia, come pure la presenza di altri disturbi del sonno, tra cui i disturbi respiratori di tipo ostruttivo, russamento ed apnee o la sindrome delle gambe senza riposo. E’ bene però ricordare come l’insonnia, nella MP, sia anche intrinsecamente legata alla patologia principale, poiché la neurodegenerazione coinvolge strutture anatomiche e neurotrasmettitori cruciali per la regolazione del ritmo sonno-veglia, interrompendo la rete che, dal tronco dell’encefalo alla corteccia, genera il sonno. Il sesso femminile, la durata di malattia e la presenza di depressione e/o ansia sono gli altri fattori correlati all’insonnia nei pazienti con MP. L’insonnia deve essere diagnosticata attraverso la raccolta anamnestica dettagliata, con l’eventuale ausilio di questionari validati che, benchè non specifici per l’insonnia nella MP, ne indagano i sintomi associati. L’actigrafia consente un monitoraggio prolungato del ritmo-sonno veglia; la video-polisonnografia notturna permette l’indagine completa delle caratteristiche del sonno del paziente con MP. Se l’insonnia è secondaria, perché dovuta a sintomi motori notturni, il trattamento terapeutico non differisce da quello dei sintomi diurni. I farmaci a lunga durata d’azione, in particolare i dopaminoagonisti, sono efficaci nel migliorare l’insonnia nei pazienti con MP, anche se l’insonnia è elencata tra i loro effetti collaterali. Studi randomizzati hanno dimostrato che Z-drug e melatonina migliorano la qualità del sonno dei pazienti con MP. Se l’insonnia non è né iatrogena né dovuta a sintomi motori, è bene ricorrere alla terapia cognitivo-comportamentale.
