Questo argomento è di crescente importanza dal momento che circa il 20% dei casi di ictus è da ricondurre alla presenza di Fibrillazione Atriale. Il trattamento in prevenzione primaria di questa patologia è la terapia anticoagulante, che se da una parte riduce il rischio tromboembolico, dall’altra comporta un incremento di rischio emorragico sistemico. Negli ultimi dieci anni l’approccio terapeutico alla fibrillazione atriale non valvolare ha subito una svolta con l’introduzione dei NAO, che hanno permesso una migliore gestione farmacologica rispetto agli anticoagulanti classici, sia in termini di profilo di rischio che di compliance del paziente. Nel corso della fase acuta dell’ictus alla luce dell’elevato rischio di infarcimento emorragico è mandatoria la temporanea sospensione della terapia anticoagulante. Risulta di crescente importanza l’individuazione di una corretta strategia per il timing di reinserimento dell’anticoagulante nei pazienti affetti da FA. Questa scelta risulta cruciale in quanto deve inserirsi nel delicato equilibrio tra rischio protrombotico e rischio emorragico.
