I gliomi sono una classe di tumori primitivi del sistema nervoso centrale dalla prognosi molto varia in relazione al loro profilo molecolare. Tuttavia il glioma di gran lunga più frequente nell’adulto è il glioblastoma, un tumore estremamente aggressivo con una prognosi che va in media dai 12 ai 18 mesi nonostante le terapie. Benchè si tratti di un argomento di nicchia, trattandosi di patologie rare, il tema del fine vita nei pazienti affetti da glioma racchiude diversi aspetti di natura assistenziale e di natura bioetica (relativi al fatto che spesso il paziente può andare in contro ad una perdita della capacità di discernimento e di autodeterminazione) che sono comuni a diversi ambiti della neurologia, specialmente nel campo delle malattie neurodegenerative. La differenza sostanziale in neuro-oncologia risiede nelle tempistiche estremamente rapide con cui un paziente si trova proiettato in questi argomenti, e questa probamente è la sfida più dura da affrontare per i pazienti in primis e per il medico che li assiste.
