Si tratta di malattie complesse da un punto di vista clinico, biochimico e genetico. Sono inoltre tra i più comuni disordini genetici, con una prevalenza di circa 1 caso ogni 5000 persone. I mitocondri contengono una porzione di DNA che costituisce l’1% circa del DNA cellulare, tuttavia vi sono anche numerosi geni all’interno del DNA nucleare che ne contribuiscono al corretto funzionamento. Poiché ogni cellula del nostro organismo contiene centinaia/migliaia di mitocondri, risulta ovvio che ogni organo possa essere potenzialmente affetto. Tuttavia sono proprio gli organi e i tessuti a maggior richiesta energetica ad essere affetti in modo preponderante, come ad esempio il sistema nervoso centrale. Il coinvolgimento del sistema nervoso centrale nelle malattie mitocondriali è solitamente acuto, può coinvolgere soggetti di tutte le età e spesso pone problemi di diagnosi differenziale con altre e più frequenti malattie neurologiche. Inoltre si tratta spesso di manifestazioni cliniche devastanti che contribuiscono alla morbilità e mortalità di questi pazienti.
