Nel momento in cui si verifichi una mutazione genetica a carico del DNA mitocondriale (mtDNA) o nucleare (nDNA), è possibile che ne consegua un’alterazione dei processi subordinati alla produzione di tale energia (principalmente ATP), con ripercussioni di varia gravità in termini di fenotipo clinico. L’attenzione per le mitocondriopatie primitive, principalmente miologico dato il preminente coinvolgimento a carico della muscolatura scheletrica, è in costante incremento, trattandosi in modo più ampio di patologie che potenzialmente possono interessare ogni organo e sistema, a partire dal sistema nervoso centrale e periferico, occhi, fegato, reni, sistema cardiovascolare e gastrointestinale. In aggiunta alle terapie definite come “cocktail mitocondriale” quali vitamine, aminoacidi e ubichinoni (i.e. Arginina, Riboflavina, Carnitina, Ubidecarenone), sono attualmente in studio differenti approcci terapeutici, basati su terapie eziopatologiche (es. geniche) o di gestione della cascata collaterale che origina dal difetto genetico primitivo (es. antiossidanti, terapie scavenging di rimozione di composti tossici). Obiettivo del presente podcast è quello di identificare i principali aspetti eziopatogenetici, diagnostici e di caratterizzazione clinica delle miopatie mitocondriali, con un accenno alle attuali e future prospettive terapeutiche.
