The Myth of Healthy ObesityCan obesity truly be considered “healthy” simply because blood sugar, blood pressure and cholesterol remain within normal limits?Modern medicine increasingly recognizes obesity as a major contributor to Type 2 diabetes, hypertension, fatty liver disease, sleep apnea, osteoarthritis, infertility, cardiovascular disease, stroke and even certain cancers. Yet the reassuring term “healthy obesity” continues to gain acceptance.But is the concept physiologically valid?In my recent article, The Myth of Healthy Obesity, I argue that the absence of measurable disease today does not necessarily imply absence of ongoing metabolic injury. Human physiology is remarkably adaptive. The body can compensate silently for years before clinical disease becomes manifest.A smoker with normal lung function is still exposed to harm. An alcoholic with normal liver enzymes is still vulnerable to toxicity. Similarly, overweight and obese individuals with apparently “normal” investigations may still be experiencing silent metabolic stress beneath the surface.Insulin resistance, endothelial dysfunction, chronic inflammation and visceral fat accumulation often begin long before diabetes, hypertension or cardiovascular disease become clinically evident.The issue is not merely body weight or BMI. BMI itself is an imperfect anthropometric tool. Two individuals with identical BMI may have profoundly different body composition, visceral fat burden and metabolic risk.The real concern is progressive pathological fat accumulation and chronic energy excess.Not all individuals manifest complications at the same degree of weight gain. Some develop diabetes early, while others compensate longer because of genetics, muscle mass, fat distribution or physical activity. However, variability in manifestation does not negate the biological burden of obesity.The body compensates — until it cannot.Many chronic diseases may in fact represent downstream manifestations of the same upstream physiological overload.The danger of the phrase “healthy obesity” is that it may unintentionally reassure people into delaying preventive action. Waiting until overt diabetes, hypertension or cardiovascular disease appears may be akin to waiting for structural failure before repairing a weakening bridge.Importantly, this discussion should never be interpreted as moral judgment. Obesity is not a character flaw. Human biology evolved for scarcity, not for continuous exposure to calorie-dense ultra-processed foods, sedentary lifestyles, sleep deprivation and chronic stress.
